目的 分析利培酮治疗首次发病(以下简称首发)的精神分裂症患者疗效与儿茶酚胺氧位甲基转移酶(COMT)基因多态性的关联,探讨利培酮疗效的敏感基因.方法 对203例首发精神分裂症患者给予利培酮治疗8周(2~8 ms/d),分别于治疗前和治疗第2~8周末采用阳性和阴性症状量表(PANSS)评分,以减分率评定疗效.并采用限制性片段长度多态技术和序列特异性引物聚合酶链反应技术测定基因型.对痊愈组(29例)、进步组(153例)及无效组(21例)精神分裂症患者与COMT基因单核苷酸多态位点(SNPs)进行关联分析.结果 COMT基因3种多态性基因型及基因频率在利培酮不同疗效患者组间分布差异均无统计学意义(P均>0.05).COMT的3个单核苷酸多态位点单体型分析,利培酮治疗痊愈组与进步组、有效组与无效组间的差异无统计学意义(P>0.05).结论 COMT基因可能不是首发精神分裂症患者抗精神病药利培酮的疗效敏感基因.

Objective To analyze the correlation between therapeutic effect of antipsychotic agent risperidone and catechol-O-methyltransferase(COMT)gene polymorphisms in first-episode schizophrenics,to further study the mechanisms of psychiatric symptoms,and to identify genes sensitive to risperidone medication.Methods 203 first-episode schizophrenics were treated with risperidone(2~8 ms/day)for 8 weeks. Reduction rate in PANSS total score from baseline to week 8 Was used for efiectiveness evaluation.Patients were divided into 3 groups based on reduction rate of PANSS total score,i.e.,remitted (≥75%,n=29),improved(75%~25%,n=153)and unresponsive(≤25%,n=21).Restriction fragment length polymorphism analysis as well as sequence-special primers PCR amplification technique were used to determine the genotypes.Results There Was on statistically significant difference in the 3 COMT polymorphism genotypes as well as allele frequency between the 3 schizophrenic groups with different responsiveness to risperidone(P>0.05).Haplotype analysis of the 3 COMT single nucleotide polymorphisms showed no statistically significant difference among remitted group,improved group and unresponsive group.Condusions COMT gene is unlikely a sensitive gene determining the therapeutic effect of antipsychotic agent risperidone in schizophrenia.
