目的观察高压氧(HBO)在小鼠急性胰腺炎中的作用。方法建立胰腺炎小鼠模型,随机分为高压氧治疗组和对照组。高压氧治疗组中,小鼠模型建立6h后给予100％氧,2.5个大气压治疗90 min,之后每12h给予治疗1次。第7天处死存活的小鼠。进行胰腺炎的严重程度进行分级肉眼及镜下分析,计算肺的湿、干重来评价肺水肿。比较两组间的差异。结果治疗组中肉眼及显微镜下的胰腺炎的严重程度评分8.2±0.8及9.5±0.3,明显低于对照组10.6±0.6及11.2±0.3(P＜0.05)。治疗组胰腺坏死表现也明显较对照组减少,分别为(41±5)％及(55±5)％(P＜0.01)。两组的肺水肿差异无统计学意义(P＞0.05)。治疗组7d存活率明显高于对照组,分别为(39±3)％和(26±1)％(P＜0.05)。结论高压氧治疗可以降低胰腺炎的坏死程度,提高重症急性胰腺炎的生存率。
목적 관찰고압양(HBO)재소서급성이선염중적작용。방법 건립이선염소서모형,수궤분위고압양치료조화대조조。고압양치료조중,소서모형건립6h후급여100％양,2.5개대기압치료90 min,지후매12h급여치료1차。제7천처사존활적소서。진행이선염적엄중정도진행분급육안급경하분석,계산폐적습、간중래평개폐수종。비교량조간적차이。결과 치료조중육안급현미경하적이선염적엄중정도평분8.2±0.8급9.5±0.3,명현저우대조조10.6±0.6급11.2±0.3(P＜0.05)。치료조이선배사표현야명현교대조조감소,분별위(41±5)％급(55±5)％(P＜0.01)。량조적폐수종차이무통계학의의(P＞0.05)。치료조7d존활솔명현고우대조조,분별위(39±3)％화(26±1)％(P＜0.05)。결론 고압양치료가이강저이선염적배사정도,제고중증급성이선염적생존솔。
Objective To study the therapeutic effect of hyperbaric oxygen (HBO) in a rat model of severe acute pancreatitis (SAP). Methods Sixty-four rats were induced with severe pancreatitis using 4％ sodium taurocholate and randomized to HBO treatment group or control group. HBO was commenced 6h after induction ( 100％ oxygen at 2. 5 atmospheres for 90 min) and continued every 12 h for a maximum of eight treatment episodes. Surviving animals were killed at 7th day. Severity of pancreatitis was graded macroscopically and microscopically. Results Macroscopic and microscopic severity scores of HBO-treated animals with pancreatitis were lower than those of controls[( 8. 2 ± 0. 8) vs ( 10. 6 ± 0. 6), and (9. 5 ±0. 3 ) vs ( 11.2 ± 0. 3 ), P ＜ 0. 05]. The HBO-treated group had reduced pancreatic necrosis compared to controls[(41 ± 5 ) ％ vs (55 ± 5 ) ％ , P ＜ 0. 01]. There was no difference in pulmonary edema between the groups ( P ＞ 0.05 ). Day-7 survival was significantly improved in the HBO-treated animals compared to controls[(39 ±3)％ vs (26 ± 1)％ ,P ＜0. 05]. Conclusion HBO therapy reduces overall severity, decreases the extent of necrosis, and improves survival in SAP.
