目的探讨狼疮性肾炎(LN)患者淋巴细胞亚群的变化及所处的不同状态在 LN发病机制中的作用。方法采用双标染色技术结合流式细胞术,对 32例 LN进行研究。结果①LN患者 CD3+CD4+细胞的百分率明显下降,CD3+CD8+细胞的百分率明显增高,CD3+CD4+/CD3+CD8+的比例<1(与对照组相比较 ,P<0.05)。②LN患者 CD45RA+细胞、CD4+CD45RA+细胞、CD4+CD45RO+细胞和 CD4+HLA-DR+细胞的百分率下降(与对照组相比较 ,P<0.05或P<0.001),HLA-DR+细胞、CD3+HLA-DR+细胞、CD19+HLA-DR+细胞和 CD19+CD43+细胞的百分率明显增高(与对照组相比较,P<0.05或P<0.01)。结论 LN患者免疫功能的紊乱与淋巴细胞活化状态及其亚群之间的平衡失调密切相关。

목적탐토랑창성신염(LN)환자림파세포아군적변화급소처적불동상태재 LN발병궤제중적작용。방법채용쌍표염색기술결합류식세포술,대 32례 LN진행연구。결과①LN환자 CD3+CD4+세포적백분솔명현하강,CD3+CD8+세포적백분솔명현증고,CD3+CD4+/CD3+CD8+적비례<1(여대조조상비교 ,P<0.05)。②LN환자 CD45RA+세포、CD4+CD45RA+세포、CD4+CD45RO+세포화 CD4+HLA-DR+세포적백분솔하강(여대조조상비교 ,P<0.05혹P<0.001),HLA-DR+세포、CD3+HLA-DR+세포、CD19+HLA-DR+세포화 CD19+CD43+세포적백분솔명현증고(여대조조상비교,P<0.05혹P<0.01)。결론 LN환자면역공능적문란여림파세포활화상태급기아군지간적평형실조밀절상관。
Aim To explor the distribution of lymphocytic subset and activated state of lymphocyte in peripheral blood from the patients with lupus nephritis(LN). Methods The immunophenotypes in peripheral blood lymphocytes of 32 LN patients and 40 healthy volunteers were analysed by flowcytometor with double-labeling using anti-CD3, anti-CD4, anti-CD8, anti-CD19, anti-CD43, anti-CD45RA, anti-CD45RO anti-HLA-DR antibodies. Results ① The percentage of CD3+ CD4+ subset decreased significantly, meanwhile the percentage of CD3+ CD8+ subset increased; the ratio of CD3+ CD4+ /CD3+ CD8+ subset was less than 1(P〈 0.05 in contrast to control).② The percentages of CD45R+ , CD4+ CD45RA+ , CD4+ CD45RO+ and CD4+ HLA-DR+ subsets in LN patients decreased (P〈 0.05 or P〈 0.01 respectively, as compared with control group); the percentages of HLA-DR+ , CD3+ HLA-DR+ , CD19+ HLA-DR+ subset and CD19+ CD43+ subset increased(P〈 0.05 or P〈 0.01 respectively, as compared with control group). Conclusion The dysfunction of immune system in the LN patients may be related to the activation of lymphocytes and imbalance among the lymphocytic subsets.
