目的:探索利用行为法对42天龄婴儿进行听力筛查的可行性。方法:在背景噪声为50±3dB(SPL,A)环境,使用作者自行研制的BA-33型听力筛查仪和电耳镜对195名北京市区38-93天的要儿进行检查。对于听行为反应阴性和检查顺序编号个位为5的受检要儿进行畸变产物耳声发射检查。使用录像机记录行为检查过程。事后复查录像记录与现场记录合并进行统计。结果:对195名北京市区38-93天的要儿进行听力筛查发现,有2名未通过,发生率达1.03%.对178名"42天"听力正常要儿进行听力检测发现,受检要儿听行为反应在1 kHz,90dB和2 kHz,60dB(SPL,A)的引出率分别为:眼部反应65.7%,67.4%,头部反应25.8%,27.5%,肢体反应70.8%,73.0%,其他反应12.9%,16.9%,无反应2.8%,1.2%。结论:以肢体反应或眼部反应其中之一引出作为听行为反应引出的阳性指征,引出率可达到93%左右,可以满足听力筛查的要求。

Objective: To explore the possibility of conducting hearing screening of the 42 days old infants by behavioral methods. Method: In an environment of 50±3dB( SPL, A) background noise, with the audiometer developed by the authors, and an electric otoscope, the hearing of 195 infants aged 38 - 93 days in Beijing area were examined. Result: Behavioral hearing screenings were performed on 195 Beijing urban infants aged from 38 to 93 days. It was found that 2 of them failed to pass the screening. The incidence rate was 1.03%. Hearing measurements of 178 normal hearing infants aged from 40to 50 days were also performed. The inducing rates of their hearing induced behavioral responses by 1 kHz 90 dB and 2 kHz 90 dB were eye responses: 65.7% , 67.4% , bead responses: 25.8% , 27.5%, limb responses: 70.8%, 73.0%, other responses: 12.9% , 16.9% , and no response: 2.8%, 1.2% respectively. Conclusion: If one of the eye response or the limb response was used as the positive index of the hearing induced behavioral response, the inducing rate might be as high as 93%. This implied that it might be used in the hearing screening.
