目的探讨改良结肠肛管吻合术在低位直肠癌治疗中的价值。方法回顾分析37例低位直肠癌的术后并发症及疗效。结果术后发生吻合口瘘2例，吻合口狭窄9例，无手术死亡和肛门失禁；盆腔复发5例。结论改良结肠肛管吻合术在保留肛门功能的同时，避免了预防性横结肠造口，其局部复发率与低位前切除和经腹会阴切除相似。
목적탐토개량결장항관문합술재저위직장암치료중적개치。방법회고분석37례저위직장암적술후병발증급료효。결과술후발생문합구루2례，문합구협착9례，무수술사망화항문실금；분강복발5례。결론개량결장항관문합술재보류항문공능적동시，피면료예방성횡결장조구，기국부복발솔여저위전절제화경복회음절제상사。
Objective　To evaluate the therapeutic effects of modified coloanal anastomosis for low rectal carcinoma.Methods　 postoperative comp lications and effects of 37 low rectal cancer patients were retrospectively stud ied.Results　Two patients encounte red anastomo tic leakage after operation,9 developed anastomotic stenosis,there were no opera tive death and incontinence;five patients had pelvic recurrences.Conclus ions　Modified coloanal anastomosis preserves sphincter function and avo ides the preventive transversostomy;local recurrence rates after operation are a like to that of anterior resection or abdominoperineal resection.
