目的研究硬-腰联合麻醉在盆腔手术中的临床疗效.方法我院2010年11月至2012年1月收治的140例盆腔手术患者随机分为联合组和对照组各70例,联合组行硬-腰联合麻醉,70例行硬膜外连续麻醉.观察记录两组患者术中脉压差、心率、呼吸变化、起效时间和不良反应.结果联合组麻醉效果显著优于对照组(P＜0.05),能更有效地控制脉压、心率和呼吸；联合组麻醉起效时间和觉醒时间均短于对照组,差异有显著性(P＜0.05);对照组术中出现11例麻醉中毒,术后出现13例手脚麻木、嗜睡,联合组术中出现1例麻醉中毒,无术后手脚麻木病例.结论硬-腰联合麻醉能有效控制患者术中血压、心率和呼吸,且具有麻醉用药剂量小、起效快、苏醒快等特点,适用于盆腔手术.
목적 연구경-요연합마취재분강수술중적림상료효.방법 아원2010년11월지2012년1월수치적140례분강수술환자수궤분위연합조화대조조각70례,연합조행경-요연합마취,70례행경막외련속마취.관찰기록량조환자술중맥압차、심솔、호흡변화、기효시간화불량반응.결과연합조마취효과현저우우대조조(P＜0.05),능경유효지공제맥압、심솔화호흡；연합조마취기효시간화각성시간균단우대조조,차이유현저성(P＜0.05);대조조술중출현11례마취중독,술후출현13례수각마목、기수,연합조술중출현1례마취중독,무술후수각마목병례.결론 경-요연합마취능유효공제환자술중혈압、심솔화호흡,차구유마취용약제량소、기효쾌、소성쾌등특점,괄용우분강수술.
Objective To observe the clinical efficacy of combined spinal-epidural anesthesia (CSEA) for pelvic surgery.Methods 140 cases of pelvic surgery patients admitted to the hospital from November 2010 to January 2012,were randomly divided into two groups:70 cases in the combined group,got the CSEA,and 70 cases in the control group,took continuous epidural anesthesia.We observed and recorded the pulse pressure,heart rate,respiratory changes,onset time and adverse reactions of two groups.Results Anesthetic effect of the combination group was significantly better than that of the control group to more effectively control the pulse pressure,heart rate and breathing; the difference was statistically significant (P＜0.05); there was less anesthetic complication in the combination group,and the onset time and wake time were also shorter than those in the control group (P＜0.05).Conclusions CSEA can effectively control blood pressure,heart rate and breathing in patients undergoing small dose of narcotic medication,rapid onset,rapid recovery,suitable for pelvic surgery.
