目的探讨一种新的后路椎板减压方式-腰椎下半椎板揭盖回植法的临床运用效果.方法 30例退行性腰椎疾病患者行后路手术,显露腰双侧椎板后在椎板脊近端大约5～6mm处用骨刀垂直于脊柱的轴线横断椎板,下半椎板游离从而方便后续的减压、融合的手术操作,术毕将修正后的椎板覆盖原位再行周定.结果手术顺利、无神经损伤等并发症,6个月后复查,所有患者覆盖后的椎板愈合良好,30例中治愈23例,显效5例,有效2例,无效O例,JOA评分术后较术前明显改善(P＜0.05).结论下半椎板揭盖回植技术安全有效,有利于术者进行彻底的神经减压、椎间融合手术操作,回植后后路结构保存,该技术是腰椎疾病后路手术中减压的良好方法.

Objective To study the clinical effect of inferior half lamina uncover-recovered technique in posterior surgery of lumbar degenerative diseases.Methods 30 patients with lumbar degenerative diseases were treated with posterior operation.Inferior half lamina was intersected from 5-6 mm distance to upper lamina ridge,so it was convenient for decompression and intervertebral fusion.After that,the intersected lamina was recovered to its former position.Results The operation was applied safely and efficiently.No serious complication was observed.After 6 months followed-up,27 cases were cured,5 cases were notable valid,and 2 cases were valid.The JOA scores of post operation were improved significantly(P＜0.05).Conclusion Inferior half lamina uncover-recovered technique is safe,effective and beneficial for nerve root decompression in poster operaperation of lumbar degeneration diseases.
