目的 探讨梗阻性结肠肿瘤一期手术切除吻合的疗效.方法 回顾性分析38例结肠肿瘤肠梗阻行一期切除吻合患者的临床资料.结果 右半结肠肿瘤梗阻18例及左半结肠肿瘤梗阻20例行一期切除吻合,两组患者平均住院时间(32±12)d和(33±10)d；并发症率为22.2％和30.0％；病死率为0％和5.0％,两组比较差异均无统计学意义(P＞0.05).结论 对于梗阻性结肠肿瘤患者应采用积极的治疗态度,创造手术条件,选择手术时机,采用病灶切除一期吻合可以达到满意的效果.

목적 탐토경조성결장종류일기수술절제문합적료효.방법 회고성분석38례결장종류장경조행일기절제문합환자적림상자료.결과 우반결장종류경조18례급좌반결장종류경조20례행일기절제문합,량조환자평균주원시간(32±12)d화(33±10)d；병발증솔위22.2％화30.0％；병사솔위0％화5.0％,량조비교차이균무통계학의의(P＞0.05).결론 대우경조성결장종류환자응채용적겁적치료태도,창조수술조건,선택수술시궤,채용병조절제일기문합가이체도만의적효과.
Objective To study the effect of primary excision and anastomosis on colon tumor complicated with bowel obstruction.Methods The clinical data of 38 patients underwent emergency one-stage resection and anastomosis of acute complete obstruction for left and right colonic carcinoma were retrospectively analyzed.Results Eighteen patients with obstructive carcinoma of the right colon were operated,the mortality was 0,the rate of complication was 22.2％,the mean hospital stay was(32 ± 12)day.Twenty patients with obstructive carcinoma of the left colon were operated,the mortality was 5.0％,the rate of complication was 30.0％,the mean hospital stay was(33 ± 10) day.There was no significant difference in mortality or the rate of complication between the two groups ( P ＞ 0.05).Conclusions Primary excision and anastomosis for colon tumor complicated with bowel obstruction is feasible and effective.
