血吸虫肝纤维化主要由宿主对虫卵及其引起的肉芽肿产生免疫应答所致,整个过程主要有中性粒细胞、嗜酸性粒细胞、B细胞、T细胞、肝星形细胞(HSC)等多种细胞参与,并受到TNF-α、IL-1、IL-6、IFN γ等Th1型和IL-4、IL-5、IL-10、IL-13等Th2型细胞因子及一些趋化因子的调控.其反应过程十分复杂,目前对于一些细胞因子的具体作用还不明确.此文通过总结血吸虫肉芽肿和肝纤维化的细胞调控和细胞因子及趋化因子调控,进一步明确血吸虫肝纤维化的病理机制,并提出新的研究方向,为未来的治疗提供参考.

Hepatic fibrosis in schistosomiasis is mainly caused by the host immune response to the schistosome eggs and the granuloma induced by eggs.Neutrophils,eosionphils,B cells,T cells,hepatic satellite cells(HSCs),some Th1 cytokines such as TNF-α,IL-1,IL-6,IFN γ,Th2 cytokines such as IL-3,IL-5,IL-10,IL-13 and chemokine are involved in the progress.The whole response is quite complex and the functions of many involved factors are uncertain.In the article,the schistosome granulomas,cell regulation,cytokine and chemokine regulation of hepatic fibrosis are summarized to further clarify the pathogenesis of liver fibrosis in schistosomiasis and new research directions are proposed to provide a reference for future treatment.
