目的 探讨甲状腺球蛋白抗体(TGAb)和甲状腺微粒体抗体(TMAb)定量检测在自身免疫性甲状腺炎诊断中的应用.方法 对219例甲状腺肿大患者和500例正常血清用酶免疫法(EIA)定量检测甲状腺球蛋白抗体和甲状腺微粒体抗体.结果 甲状腺肿大患者TGAb和TMAb定量检测结果分别为(295±648)IU/mL和(309±536)IU/mL,检出率分别为61%(134/219)和62%(136/219),经过2种抗体的检测,最后有126例确诊为自身免疫性甲状腺炎(AITT).500例健康对照TGAb和TMAb定量检测分别为(9±4)IU/mL和(14±6)IU/mL.结论 EIA定量检测血清TGAb和TMAb操作简便,结果准确,符合临床常规使用要求,对自身免疫性甲状腺炎的诊断、疗效观察具有重要临床应用价值.

Objective To explore the clinical application of thyroglobulin antibody (TGAb) and thyroid microsomal antibody (TMAb) quantitive determination with enzyme immunoassay (EIA) in the diagnosis of autoimmune thyroiditis.Methods EIA was applied to quantitively determining serum TGAb and TMAb in 219 patients with thyromegaly (thyromegaly group) and 500 healthy controls (healthy control group).Results The serum level of TGAb and TMAb in thyromegaly group was (295±648) IU/mL [detection rate: 61%(134/219)] and (309±536)IU/mL [detection rate: 62% (136/219)], respectively. Eventually, 126 cases were definitely diagnosed as autoimmune thyroiditis (AITT) through the determination of the two antibodies. The serum level of TGAb and TMAb in healthy control group was (9±4) IU/mL and (14±6) IU/mL respectively.Conclusion TGAb and TMAb quantitive determination with EIA is convenient and accurate, in accordance with clinical requirement, contributing to diagnosis and therapeutic effect evaluation of autoimmune thyroiditis.
