目的探讨经会阴尿道吻合术治疗男童急性外伤性后尿道断裂的效果.方法总结1994至2007年我院采用经会阴尿道吻合术修复男童急性完全性尿道断裂共24例资料,年龄4～15岁,平均6.5岁,本组后尿道损伤均因骨盆骨折导致,在牛命体征平稳的情况下,手术时问为外伤后10 h至7 d.经术前自尿道外口造影诊断后尿道完全性断裂.所有病例均急诊行经会阴尿道端端吻合术.术后留置导尿管2～4周.结果 24例新鲜尿道外伤除1例手术后有尿道狭窄,手术后3个月做内切开;另外1例手术后随诊有轻微压力性尿失禁外,其余病例一次手术均获成功,排尿良好.结论经会阴修复男童急性外伤性后尿道断裂效果满意.

Objective To evaluate the surgical procedure for fresh posterior urethral injury in young boys.Methods Twenty-four boys with fresh posterior urethral injury were treated in our hospital from 1994 to 2007.Age ranged from 4 to 15yrs (mean 6. 5yrs). Causes of injury included traffic accident in 23 cases and crush injury in 1 case.The time between injury and operation varied from 10 hours to 7 days.All patients underwent simultaneous retrograde urethrography.Ultrasonography or CT scan was used to assess the conditions of upper urinary tract End to end anastomosis via suprapubic combined with perineal approach was used in these patients with complete posterior urethral disruption. Urethral catheter was removed 2-4 weeks after operation. Results Follow-up was done by direct questioning and retrograde urethrography. Urinary flow study was used in selected older cooperative children. Twenyty-two patients can void urethrally with adequate flow, one had stress incontinence, and one patient underwent optical urethrotomy 3 months postoperation. Conclusions End to end anastomosis via suprapubic combined with perineal approach can get satisfactory results for fresh posterior urethral injury in young boys.
