目的观察克骨汤对兔激素性股骨头坏死血管内皮生长因子(VEGF)表达的影响,探讨其作用机制。方法36只新西兰白兔随机分为三组:模型+克骨汤组(简称克骨汤组)12只、模型组12只、对照组12只。在第4、8、12周分批处死实验动物,获取支配股骨头血运的主要血管(旋股内侧动脉、股静脉)及股骨头。将其制成标本,采用原位杂交法测定VEGF含量,并行常规病理组织学检查。结果第4周时,各组动脉内膜层、静脉内膜层中都有VEGF mRNA阳性染色,但各组间的差异无统计学意义(F分别=1.36、2.46,P均>0.05);第8周和第12周时,各组VEGF mRNA阳性染色的差异有统计学意义(F分别=14.58、11.35,P均<0.05);第8周时,与克骨汤组、对照组比较,模型组动脉内膜层、静脉内膜层的VEGF mRNA阳性染色减弱,差异均有统计学意义(t分别=12.13、15.23;19.36、18.25,P均<0.05);第12周时,克骨汤组与模型组、模型组与对照组比较,动脉内膜层、静脉内膜层VEGF mRNA阳性染色差异均有统计学意义(t分别=12.09、19.35;20.37、16.74,P均<0.05),但克骨汤组与对照组比较,静脉内膜层VEGF mRNA阳性染色差异无统计学意义(t=2.59,P>0.05)。结论克骨汤可上调VEGF mRNA在血管内膜中的表达,使兔激素性股骨头坏死血管壁中主要促血管生长因子生理活性增强。

목적관찰극골탕대토격소성고골두배사혈관내피생장인자(VEGF)표체적영향,탐토기작용궤제。방법36지신서란백토수궤분위삼조:모형+극골탕조(간칭극골탕조)12지、모형조12지、대조조12지。재제4、8、12주분비처사실험동물,획취지배고골두혈운적주요혈관(선고내측동맥、고정맥)급고골두。장기제성표본,채용원위잡교법측정VEGF함량,병행상규병리조직학검사。결과제4주시,각조동맥내막층、정맥내막층중도유VEGF mRNA양성염색,단각조간적차이무통계학의의(F분별=1.36、2.46,P균>0.05);제8주화제12주시,각조VEGF mRNA양성염색적차이유통계학의의(F분별=14.58、11.35,P균<0.05);제8주시,여극골탕조、대조조비교,모형조동맥내막층、정맥내막층적VEGF mRNA양성염색감약,차이균유통계학의의(t분별=12.13、15.23;19.36、18.25,P균<0.05);제12주시,극골탕조여모형조、모형조여대조조비교,동맥내막층、정맥내막층VEGF mRNA양성염색차이균유통계학의의(t분별=12.09、19.35;20.37、16.74,P균<0.05),단극골탕조여대조조비교,정맥내막층VEGF mRNA양성염색차이무통계학의의(t=2.59,P>0.05)。결론극골탕가상조VEGF mRNA재혈관내막중적표체,사토격소성고골두배사혈관벽중주요촉혈관생장인자생리활성증강。
Objective To investigate the effect of Kegu recipe on angiogenesis stimulating factors (VEGF mRNA)in the steroid-induced avascular ecrosis of the rabbit femoral head. Methods A total of 36 adult New Zealand rabbits were randomly divided into three groups equally: the control group, the model group, and the Kegu recipe group. The experimental animals were partial executed at fourth, eighth, twelfth weeks so that to access the main blood vessels of the femoral head including the medial femoral circumfles artery, femoral medial femoral circumflex artery and femoral vein. The specimen used the in situ hybridization method to measure VEGF levels and underwent routine histologic examination. Results At fourth week, there was no statistical significance between three groups though all groups showed positive staining in VEGF mRNA of endarterium and vein intima(F=1.36,2.46,P>0.05). At eighth and twelfth week, the differences of VEGF mRNA positive staining in endarterium and vein intima were statistical significance (F=14.58,11.35,P<0.05). At eighth week, the VEGF mRNA positive staining in endarterium and vein intima of model group was significantly lower than Kegu recipe group and control group(t=12.13,15.23,19.36,18.25,P<0.05). At twelfth week, the differences of the VEGF mRNA positive staining in endarterium and vein intima were statistical significance between model group and control group as well as between Kegu recipe group and model group(t=12.09,19.35,20.37,16.74,P<0.05). The differences of the VEGF mRNA positive staining in vein intima between Kegu recipe group and control group had no statistical significances (t=2.59,P>0.05). Conclusions Kegu recipe could improve the presence of VEGF mRNA, which may boost the biological activity of the major factors for angiogenesis in the vascular wall of the steroid-induced necrosis of the femoral head.
