目的:评价血浆催乳素( PRL)水平与绝经后女性乳癌危险性的关系。方法:采用化学发光法测定83例绝经后女性乳癌病例和85例对照的血浆PRL水平,应用logistic回归分析绝经后血浆PRL水平与乳癌危险性的关系。根据肿瘤临床病理特征(肿瘤大小、病理类型、肿瘤分级以及ER是否阳性)对病例组进行分组,评估血浆PRL水平与不同乳癌亚组危险性的关系。结果:①病例组血浆PRL水平高于对照组(t=2.104,P=0.037)。②以血浆PRL<P25为非暴露参考,血浆PRL P25~水平调整OR(95%CI)为1.04(1.01~2.83),血浆PRL P50~水平调整OR (95%CI)为1.38(1.26~3.83),血浆PRL≥P75水平调整OR(95%CI)为1.62(1.38~4.14),χ2趋势=4.986,P=0.027。③PRL与绝经后乳癌危险性的关系在ER+的肿瘤中略增强(χ2异质性=5.127,P=0.024),在不同肿瘤大小、病理类型、肿瘤分级间无明显变化(P均>0.05)。结论:血浆PRL水平与绝经后女性乳癌的危险性呈正相关,尤其是ER+的乳癌。

목적:평개혈장최유소( PRL)수평여절경후녀성유암위험성적관계。방법:채용화학발광법측정83례절경후녀성유암병례화85례대조적혈장PRL수평,응용logistic회귀분석절경후혈장PRL수평여유암위험성적관계。근거종류림상병리특정(종류대소、병리류형、종류분급이급ER시부양성)대병례조진행분조,평고혈장PRL수평여불동유암아조위험성적관계。결과:①병례조혈장PRL수평고우대조조(t=2.104,P=0.037)。②이혈장PRL<P25위비폭로삼고,혈장PRL P25~수평조정OR(95%CI)위1.04(1.01~2.83),혈장PRL P50~수평조정OR (95%CI)위1.38(1.26~3.83),혈장PRL≥P75수평조정OR(95%CI)위1.62(1.38~4.14),χ2추세=4.986,P=0.027。③PRL여절경후유암위험성적관계재ER+적종류중략증강(χ2이질성=5.127,P=0.024),재불동종류대소、병리류형、종류분급간무명현변화(P균>0.05)。결론:혈장PRL수평여절경후녀성유암적위험성정정상관,우기시ER+적유암。
To estimate the association of plasma prolactin level with risk of breast cancer among postmenopausal women.Methods:By case-control study,the level of plasma prolactin was measured in 83 postmenopausal breast cancer patients and 85 controls by chemiluminescence .Logistic regression models were used to examine association of prolactin with breast cancer risk .In addition , we estimated the association across various case groups ( ductal vs lobular , tumor size≤2 cm vs >2 cm, ER+vs ER-, well/moderately differentiated vs poorly differentiated ) .Results:The plasma prolactin level of case group was significantly higher than that of control group (t=2.104,P=0.037).Increased risk of breast canc-er was positively associated with elevated plasma level of prolactin [ with <P25 of plasm prolactin level as unexposure refer-ence, OR(95%CI) for P25 -,P50 -,and ≥P75 was 1.04(1.01-2.83),1.38(1.26-3.83),and 1.62(1.38-4.14),χ2trend =4.986,P=0.027].The association was increased among ER +cancer(χ2heterogeneity =5.127,P=0.024).Conclu-sion:It is suggested that high plasma level of prolactin is positively associated with increased breast cancer risk among postmenopausal women , especially for ER +cancer .
