目的：观察加味半夏白术天麻汤治疗高血压左心室肥厚的疗效及安全性。方法：将高血压左心室肥厚患者93例随机分为治疗组47例、对照组46例。对照组给予坎地沙坦片4～8 mg/次,2次/d。治疗组在对照组治疗的基础上予加味半夏白术天麻汤，1剂/d，分3次口服；2组均以治疗2个月为1个疗程，3个疗程后观察治疗前后血压、临床症状、左心室肥厚等指标,并测血、尿、便常规,肝肾功能等,同时记录不良反应。结果：治疗组血压降低平均值为10/6 mmHg,与对照组比较差异无统计学意义(P＞0.05)。超声心动图显示左心室肥厚疗效方面治疗组显效13例，有效28例，总有效率87.23%；对照组显效11例，有效22例，总有效率71.74%，2组总有效率比较差异有统计学意义(P＜0.05)。眩晕、头痛、胸闷憋气等中医证候疗效方面治疗组明显优于对照组，中医辨证论治优势明显(P＜0.01)，治疗期间无明显不良反应发生。结论：加味半夏白术天麻汤在平稳降压的同时能明显改善患者临床症状，逆转左心室肥厚。
목적：관찰가미반하백술천마탕치료고혈압좌심실비후적료효급안전성。방법：장고혈압좌심실비후환자93례수궤분위치료조47례、대조조46례。대조조급여감지사탄편4～8 mg/차,2차/d。치료조재대조조치료적기출상여가미반하백술천마탕，1제/d，분3차구복；2조균이치료2개월위1개료정，3개료정후관찰치료전후혈압、림상증상、좌심실비후등지표,병측혈、뇨、편상규,간신공능등,동시기록불량반응。결과：치료조혈압강저평균치위10/6 mmHg,여대조조비교차이무통계학의의(P＞0.05)。초성심동도현시좌심실비후료효방면치료조현효13례，유효28례，총유효솔87.23%；대조조현효11례，유효22례，총유효솔71.74%，2조총유효솔비교차이유통계학의의(P＜0.05)。현훈、두통、흉민별기등중의증후료효방면치료조명현우우대조조，중의변증론치우세명현(P＜0.01)，치료기간무명현불량반응발생。결론：가미반하백술천마탕재평은강압적동시능명현개선환자림상증상，역전좌심실비후。
Objective:To observe clinical effects and the safety of modified BanXia BaiZhu TianMaTang in the treatment for hypertension left ventricular hypertrophy (LVH). Method: All 93 patients were randomized into 47 cases in the treatment group and 46 cases in the control group. The control group took candesartan cilexetil tablets, 4 to 8 mg each time, twice per day. The treatment group were administered with modified BanXia BaiZhu TianMa Tang, one dose per day, three times one day. Two months were one course of treatment, blood pressure, clinical symptoms and the indexes of LVH were investigated before and after treating in three courses of treatment, as well routine blood test, routine urine test and routine stool test, liver and kidney functions, at the same time, adverse reactions were recorded. Result:Average decrease of blood pressure in the treatment group was 10/6 mmHg, it had no statistical meaning compared with that in the control group (P>0.05). The results of ultrasound cardiogram showed that there were 13 cases markedly effective, 28 cases effective, total effective rate of the treatment group was 87.23%;there were 11 cases remarkably effective, 22 cases effective, total effective rate 71.74%. The difference had statistical meaning in total effective rate between both groups (P<0.05). The treatment group was superior to the control group notably in vertigo, headache, chest stuffiness and short of breath, which demonstrated the advantages of syndrome differentiation and treatment (P<0.05). There was no obvious adverse reaction in the course of treatment. Conclusion: Modified BanXia BaiZhu TianMaTang could improve clinical symptoms and LVH of the patients significantly, it is helpful to decrease blood pressure.
