目的:探讨2型糖尿病(T2DM)患者糖尿病肾病发生与发展的相关危险因素。方法对264例T2DM患者据24 h尿白蛋白排泄率分为正常白蛋白尿组(DN0组88例),微量白蛋白尿组(DNl组96例),大量白蛋白尿组(DN2组80例),并进行临床分析。结果①DN2组的舒张压(DBP)、病程、收缩压(SBP)、甘油三脂(TG)显著高于DN0组(P<0.05),DN1组的SBP、DBP显著高于DN0组(P<0.05)。DN2组的病程、SBP、HbA1C、TG显著高于DN1组(P<0.05)。②Logistic回归分析表明:病程、DBP是DN发生的危险因素,病程、TG是DN发展的危险因素。结论病程是T2DM患者DN发生和发展的危险因素,严格控制患者的血脂和血压有助于延缓糖尿病肾病的发生与发展。

목적:탐토2형당뇨병(T2DM)환자당뇨병신병발생여발전적상관위험인소。방법대264례T2DM환자거24 h뇨백단백배설솔분위정상백단백뇨조(DN0조88례),미량백단백뇨조(DNl조96례),대량백단백뇨조(DN2조80례),병진행림상분석。결과①DN2조적서장압(DBP)、병정、수축압(SBP)、감유삼지(TG)현저고우DN0조(P<0.05),DN1조적SBP、DBP현저고우DN0조(P<0.05)。DN2조적병정、SBP、HbA1C、TG현저고우DN1조(P<0.05)。②Logistic회귀분석표명:병정、DBP시DN발생적위험인소,병정、TG시DN발전적위험인소。결론병정시T2DM환자DN발생화발전적위험인소,엄격공제환자적혈지화혈압유조우연완당뇨병신병적발생여발전。
Objective To study on the risk factors of diabetic nephropathy (DN) in type 2 diabetes mel itus (T2DM). Methods 264 patients with T2DM were selected and divided into normal albuminuria group(DN0 group,n=88),microalbuminuria group (DN1 group,n=96)and macroalbuminuria group(DN2 group,n=80)and analyze the clinical data. Results (1) The diastolic blood pressure(DBP), diabetic duration, systolic blood pressure(SBP),triglyceride(TG) in DN2 were higher than DN0 ; the SBP and DBP in DN1 were significantly higher than DN0;the diabetic duration, SBP, Glycosylated hemoglobin A1C (HBA1C) ,TG in DN2 were significantly higher than DNl. (2) Logistic regression analysis showed that:The diabetic duration and DBP were risk factors of DN development . The DBP and TG were risk factors of DN progression. Conclusions The diabetic duration risk factor of DN development and progression in type 2 diabetic mel itus. Strict control of blood pressure and blood lipids help delay the development and progression of diabetic nephropathy.
