目的:观察应用左卡尼汀对老年维持性血液透析( maintenance hemodialysis,MHD)患者血脂及营养情况的影响。方法将72例老年MHD患者随机分为治疗组和对照组,治疗组于每次血液透析结束前从静脉缓慢注入左旋肉碱1g 加生理盐水20 ml,共6个月。对照组于结束前静脉注入生理盐水20 ml,频次及疗程同治疗组,疗程共6个月。比较两组治疗前后患者的总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、血清白蛋白(ALB)、前白蛋白(PAB)、转铁蛋白(TF)、血红蛋白(HGB)水平。结果治疗组经左旋肉碱治疗6个月后,与治疗前及对照组比较,TG、CHO、LDL水平下降,ALB、PAB、TF、Hb水平上升,其差异有统计学意义(均P<0.05)。结论左旋肉碱可改善老年维持性血液透析的血脂异常和营养不良。

목적:관찰응용좌잡니정대노년유지성혈액투석( maintenance hemodialysis,MHD)환자혈지급영양정황적영향。방법장72례노년MHD환자수궤분위치료조화대조조,치료조우매차혈액투석결속전종정맥완만주입좌선육감1g 가생리염수20 ml,공6개월。대조조우결속전정맥주입생리염수20 ml,빈차급료정동치료조,료정공6개월。비교량조치료전후환자적총담고순(TC)、감유삼지(TG)、고밀도지단백(HDL)、저밀도지단백(LDL)、혈청백단백(ALB)、전백단백(PAB)、전철단백(TF)、혈홍단백(HGB)수평。결과치료조경좌선육감치료6개월후,여치료전급대조조비교,TG、CHO、LDL수평하강,ALB、PAB、TF、Hb수평상승,기차이유통계학의의(균P<0.05)。결론좌선육감가개선노년유지성혈액투석적혈지이상화영양불량。
Objective:To study the impact of levocarnitine on lipid profile and malnutrition in elderly maintenance hemodialysis (MHD). Methods: 72 elderly patients on MHD were randomly divided into treatment group and control group. The treatment group was given levocarnitine 1g by intravenous injection at the end of each hemodialysis. The duration oftherapy was six months. The control group was given physiological saline by intravenous injection at the end of each hemodialysis. The frequency and duration of therapy were the same in the two groups. Total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein serum albumin, prealbumin, transferring, and hemoglobin were measured. Results: Total cholesterol, triglyceride, low density lipoprotein were reduced in treatment group compared with control group (P <0.05). Serum albumin, prealbumin, transferring, and hemoglobin were increased in treatment group compared with control group ( P < 0. 05). Conclusions:Levocarnitine can significantly improve malnutrition and lipid profile function in elderly hemodialysis patients.
