目的:评价新诊断T2DM患者胰岛素抵抗、尤其骨骼肌胰岛素抵抗的水平,探讨其与正常人群的差异。方法入选48例新诊断2型糖尿病患者及健康对照者,分别进入2型糖尿病组和正常对照组,检测清晨空腹状态下糖脂代谢水平、胰岛素抵抗指数HOMA-IR、骨骼肌肌纤维细胞内脂肪含量(IMCL)。结果2型糖尿病患者HOMA-IR 、比目鱼肌及胫骨前肌IMCL均较正常对照组升高(P均<0.05)。 IMCL与HOMA-IR呈较强的正相关关系,与BMI、腰臀比、Tch, TG, LDL-C, HDL-C则未见相关。结论新诊断T2DM患者存在骨骼肌胰岛素抵抗。 IMCL能独立于肥胖、脂代谢水平,反映骨骼肌胰岛素抵抗程度。

목적:평개신진단T2DM환자이도소저항、우기골격기이도소저항적수평,탐토기여정상인군적차이。방법입선48례신진단2형당뇨병환자급건강대조자,분별진입2형당뇨병조화정상대조조,검측청신공복상태하당지대사수평、이도소저항지수HOMA-IR、골격기기섬유세포내지방함량(IMCL)。결과2형당뇨병환자HOMA-IR 、비목어기급경골전기IMCL균교정상대조조승고(P균<0.05)。 IMCL여HOMA-IR정교강적정상관관계,여BMI、요둔비、Tch, TG, LDL-C, HDL-C칙미견상관。결론신진단T2DM환자존재골격기이도소저항。 IMCL능독립우비반、지대사수평,반영골격기이도소저항정도。
Objective:To investigate the feature of insulin resistance, especial y insulin resistance of skeletal muscle, in Patients with Newly Diagnosed Type 2 Diabetes Mel itus. Methods:48 patients with newly diagnosed T2DM were enrol ed as T2DM group and another 20 participants with NGT were recruited as control group.Glycemic control, lipid profile, HOMA indices, and IMCL quantified by 1H-magnetic resonance spectroscopy were tested and compared.Results:In T2DM group, the HOMA-IR and IMCL in soleus and tibilis anterior muscles were higher than those in control group (P<0.05).The correlation analyses showed that IMCL was positively correlated to HOMA-IR, but not with BMI, waist to hip ratio and lipid profile. Conclusion:Insulin resistance exists in the skeletal muscle of patients with newly diagnosed T2DM. IMCL is a credible tissue marker for skeletal muscle insulin resistance, independent of obesity and lipid profile.
