目的:探讨周围神经传导速度测定在老年糖尿病诊断中的作用。方法对我科2012年1月~2013年6月87例以周围神经病变为首发症状的老年人进行周围神经MCV、CMAP、SNAP、SNCV测定,并对MCV、CMAP、SNAP、SNCV异常者进一步行血糖测定。结果87例老年患者中神经电生理检查 MCV、CMAP、SNAP、SNCV异常率为100%,其中SNCV异常86%,SNAP下降97%,其中以正中神经最为明显。空腹血糖异常24例,餐后2 h血糖异常31例,31例患者确诊为糖尿病,占检查者35.6%。结论神经电生理检查对于无明显糖尿病症状的老年腕管综合征患者阳性率高,可作为老年隐匿性糖尿病诊断的首选检查方法之一。

목적:탐토주위신경전도속도측정재노년당뇨병진단중적작용。방법대아과2012년1월~2013년6월87례이주위신경병변위수발증상적노년인진행주위신경MCV、CMAP、SNAP、SNCV측정,병대MCV、CMAP、SNAP、SNCV이상자진일보행혈당측정。결과87례노년환자중신경전생리검사 MCV、CMAP、SNAP、SNCV이상솔위100%,기중SNCV이상86%,SNAP하강97%,기중이정중신경최위명현。공복혈당이상24례,찬후2 h혈당이상31례,31례환자학진위당뇨병,점검사자35.6%。결론신경전생리검사대우무명현당뇨병증상적노년완관종합정환자양성솔고,가작위노년은닉성당뇨병진단적수선검사방법지일。
Objective to investigate the peripheral nerve conduction velocity measurement in the diagnosis of senile diabetes. Methods In our department in January 2012- June 2013 87 patients with peripheral neuropathy symptoms to start the elderly for peripheral nerve MCV, CMAP, SNAP, SNCV determination, and the MCV, CMAP, SNAP, SNCV abnormal line further blood glucose determination. Results 87 cases of elderly patients with nerve electrophysiological examination the MCV, CMAP, SNAP, SNCV abnormal rate was 100%, the abnormal SNCV 86%, a 97% drop in the SNAP, one of the median nerve is most obvious. 24 cases of abnormal fasting plasma glucose, postprandial 2 hours blood glucose abnormalities in 31 cases, 31 cases of patients diagnosed with diabetes, 35.6% of the inspectors. Conclusion nerve electrophysiological examination for no obvious symptoms of diabetes in elderly patients with carpal tunnel syndrome positive rate is high, can be used as old occult one of the first choice for the diagnosis of diabetes.
