目的:探讨B超联合X线双定位在微创经皮肾镜碎石取石术中的临床疗效。方法:选取2010年8月-2012年10月收治的159例上尿路结石患者,采用联合B超和X线C臂定位,行微创经皮肾碎石取石术。结果:159例均穿刺成功,并建立微通道。手术时间(68±20)min,出血量(92±36)ml。一次结石碎石率97%(154/159),一次结石清除率89%(141/159)。术后感染发热10例,输血2例,无胸膜和肠道损伤等严重并发症发生。结论:B超联合X线双定位具有术中定位准确、扩张时可以准确监测扩张方向和深度等优势。行经皮肾碎石取石术安全、微创、有效,适合在具有一定医疗设备和技术的基层医院推广。

목적:탐토B초연합X선쌍정위재미창경피신경쇄석취석술중적림상료효。방법:선취2010년8월-2012년10월수치적159례상뇨로결석환자,채용연합B초화X선C비정위,행미창경피신쇄석취석술。결과:159례균천자성공,병건립미통도。수술시간(68±20)min,출혈량(92±36)ml。일차결석쇄석솔97%(154/159),일차결석청제솔89%(141/159)。술후감염발열10례,수혈2례,무흉막화장도손상등엄중병발증발생。결론:B초연합X선쌍정위구유술중정위준학、확장시가이준학감측확장방향화심도등우세。행경피신쇄석취석술안전、미창、유효,괄합재구유일정의료설비화기술적기층의원추엄。
Objective:To investigate the clinical therapeutic effect of combined B-ultrasound and X-ray guided minimally invasive percutaneous nephrolithotomy(MPCNL) in the treatment of renal pelvic and upper ureteral calculi.Method:159 cases with upper urinary tract calculi admitted to our department of urinary surgery from August 2010 to October 2012 were given MPCNL using combined B-ultrasound and X-ray guided localization.Result:Renal punctures were successful and micro-channels established in all patients.Operative time was (68±20)min,blood loss was (92±36)ml;the success rate of fragmentation on 1 session was 97%(154/159) and stone clearance rate was 89%(141/159).10 cases had postoperative urinary tract infection;2 cases had blood transfusion,and no severe complications such as pleural and intestinal damage occurred.Conclusion:Combined B-ultrasound and X-ray localization is advantage in localization accuracy and accurate monitoring of extensive direction and depth.MPCNL is safe,minimally invasive and effective,it is worth generalizing in basic level hospital with some medical equipments and technology.
