目的探讨雌激素受体α(estrogen receptor α,ERα) mRNA 和雄激素受体(androgen receptor ,AR) mRNA与肝组织病理状态的相关性。方法收集111例慢性乙型肝炎患者肝组织病理标本及外周血单个核细胞,肝组织病理诊断包括炎症程度分级及纤维化分期,外周血单个核细胞提取总RNA进行反转录,以获取的cDNA为模板,运用实时定量PCR方法测定外周血单个核细胞内 ERα mRNA 和 AR mRNA 含量。结果外周血单个核细胞中 AR mRNA的含量在不同的肝组织炎症分级和纤维化分期存在着显著差异,ERαmRNA含量在不同的肝组织炎症程度分级中有显著性差异。通过Logistic回归分析,得出AR mRNA是影响肝组织炎症分级的主要因素,年龄和AR mRNA两个主要因素会影响肝组织纤维化分期。结论 AR表达水平是影响慢性乙型肝炎的发病和进展的重要因素。

Objective To evaluate the efficacy of estrogen receptor α(ER α) mRNA and androgen receptor (AR) mRNA correlated with histopathology of liver tissue in patients with chronic hepatitis B (CHB) .Methods One hundred and one CHB patients were consecutively enrolled in this study ,liver tissues and peripheral blood mononuclear cells (PBMCs) were collected .Liver histopathological diagnosis ,including fibrosis stage and inflammatory activity grade ,was processed by more than two independent pathologists .Total RNA was isolated from PBMCs ,levels of ERαmRNA and AR mRNA from PBMCs were measured using real time quantitative polymerase chain reaction (RT-PCR) .Results The level of AR mRNA from PBMCs was significantly different among patients with different inflammatory grade and fibrosis stage of liver tissues .Similarly ,the level of ERα mRNA from PBMCs was significantly different liver inflammatory grade .Using logistic regression analysis ,it is observed that AR mRNA was the principle factor that influence the inflammatory grade of liver tissue ,while age and AR mRNA were the principle factors for fibrosis stage of liver tissue in patients with chronic hepatitis CHB .Conclusion The level of AR was an important factor to affect the onset and development progression of CHB patients .
