目的 探讨高血压合并糖尿病患者中肿瘤坏死因子α(TNF-α)、瘦素、可溶性血管内皮细胞黏附分子1(sVCAM-1)含量的变化及其意义.方法 对34例高血压病患者(高血压组)、41例高血压合并糖尿病患者(合并组)和30名健康人(正常对照组)采用双抗体夹心法酶联免疫吸附测定(ELISA)检测血清TNF-α、瘦素和sVCAM-1含量;采用全自动血生化分析仪和化学发光法检测空腹血糖(FPG)和空腹胰岛素(FINS)含量,采用稳态模型评估法(HOMA)计算胰岛素抵抗指数(HOMA-IR).结果 ①高血压组、合并组血清TNF-α、瘦素和sVCAM-1水平及HOMA-IR均明显高于正常对照组[TNF-α:(29.7 ±2.6),(32.9 ±3.8)μg/L比(26.1±2.7) μg/L;瘦素:(5.9±1.2),(12.8±2.6) μg/L比(3.2±0.7) μg/L,sVCAM-1:(841±117),(947±113) μg/L比(678±87) μg/L,HOMA-IR:3.5 ±0.6,5.7±1.1比1.3±0.5](均P<0.01);高血压组与合并组之间以上各指标比较差异均有统计学意义(均P<0.01).②高血压组、合并组TNF-α、瘦素和sVCAM-1水平与HOMA-IR呈正相关(r值分别为0.373、0.365、0.375,均P<0.05;r值分别为0.518、0.469、0.674,均P <0.05).结论 ①TNF-α、瘦素和sVCAM-1是高血压病及高血压合并糖尿病发生发展的高危因素.②TNF-α、瘦素和sVCAM-1与胰岛素抵抗明显相关.

Objective To investigate the change of serum tumor necrosis factor α(TNF-α),leptin,soluble vascular cell adhesion molecule-1 (sVCAM-1) in blood serum for essential hypertensive patients and essential hypertensive patients complicated with diabetes mellitus.Methods Seventy-five patients were divided into essential hypertension group (group B,34 cases) and essential hypertension combined with diabetes mellitus group(group C,41 cases).Thirty healthy people were chosen as normal control group.A double antibody sandwich enzyme-linked immunosorbent assay (ELISA) was developed to test the level of TNF-α,leptin and sVCAM-1 in blood serum.Automatic blood biochemical analyzer and chemiluminescence were used respectively to detect the contents of fasting serum glucose (FPG) and fasting serum insulin (FINS) of the subjects.Insulin resistance index is estimated by homeostasis model assessment.Results ①The levels of TNF-α,leptin,sVCAM-1 and HOMA insulin resistance index (HOMA-IR) in group B and C were significantly higher than those in group A[TNF-α:(29.7 ±2.6),(32.9 ±3.8) μg/L vs (26.1 ± 2.7) μg/L; leptin:(5.9 ± 1.2),(12.8 ± 2.6) μg/L vs (3.2 ± 0.7) μg/L,sVCAM-1:(841 ±117),(947 ± 113) μg/L vs(678 ±87) μg/L,HOMA-IR:3.5 ±0.6,5.7 ±1.1 vs 1.3 ±0.5] (all P<0.01),the indexes between group B and group C were significantly different as well(all P<0.01).②In group B and C,the levels of TNF-α,leptin,sVCAM-1 were positively correlated with HOMA-IR (r =0.373,r =0.365,r=0.375,allP <0.05;r=0.518,r =0.469,r =0.674,allP <0.05).Conclusions ①TNF-α,leptin,sVCAM-1 are risk factors for hypertension and hypertension complicated with diabetes mellitus.②These indexes are also significantly associated with insulin resistance.
