目的 探讨合并三角纤维软骨复合体(TFCC)损伤桡尺远侧关节不稳定、难复位型盖氏骨折的手术指征和治疗方法.方法 2008年1月至2012年6月,对12例合并TFCC损伤采用钢板固定桡骨干骨折后桡尺远侧关节不稳定难以复位的盖氏骨折患者,采用背侧辅助切口切开复位桡尺远侧关节,并采用骨锚或钻孔的方式修复TFCC,取得了良好的效果.其中男8例,女4例;年龄28~50岁,平均37.5岁.均为外伤引起,受伤时间均在1周之内.结果 术后12例患者桡骨远端骨折均愈合,随访6~24个月.按上肢功能评定标准评定,平均得分为7.3分(3.3~ 12.6分).结论 对于合并TFCC损伤、桡尺远侧关节不稳定、难以复位的盖氏骨折,采取桡骨骨折复位内固定结合TFCC修复能获得良好的效果.
目的 探討閤併三角纖維軟骨複閤體(TFCC)損傷橈呎遠側關節不穩定、難複位型蓋氏骨摺的手術指徵和治療方法.方法 2008年1月至2012年6月,對12例閤併TFCC損傷採用鋼闆固定橈骨榦骨摺後橈呎遠側關節不穩定難以複位的蓋氏骨摺患者,採用揹側輔助切口切開複位橈呎遠側關節,併採用骨錨或鑽孔的方式脩複TFCC,取得瞭良好的效果.其中男8例,女4例;年齡28~50歲,平均37.5歲.均為外傷引起,受傷時間均在1週之內.結果 術後12例患者橈骨遠耑骨摺均愈閤,隨訪6~24箇月.按上肢功能評定標準評定,平均得分為7.3分(3.3~ 12.6分).結論 對于閤併TFCC損傷、橈呎遠側關節不穩定、難以複位的蓋氏骨摺,採取橈骨骨摺複位內固定結閤TFCC脩複能穫得良好的效果.
목적 탐토합병삼각섬유연골복합체(TFCC)손상뇨척원측관절불은정、난복위형개씨골절적수술지정화치료방법.방법 2008년1월지2012년6월,대12례합병TFCC손상채용강판고정뇨골간골절후뇨척원측관절불은정난이복위적개씨골절환자,채용배측보조절구절개복위뇨척원측관절,병채용골묘혹찬공적방식수복TFCC,취득료량호적효과.기중남8례,녀4례;년령28~50세,평균37.5세.균위외상인기,수상시간균재1주지내.결과 술후12례환자뇨골원단골절균유합,수방6~24개월.안상지공능평정표준평정,평균득분위7.3분(3.3~ 12.6분).결론 대우합병TFCC손상、뇨척원측관절불은정、난이복위적개씨골절,채취뇨골골절복위내고정결합TFCC수복능획득량호적효과.
Objective To discuss the surgical indications and techniques to treat triangle fibrocartilage complex (TFCC) injury and distal radioulnar joint (DRUJ) instability in Galeazzi fracture-dislocations.Methods From January 2008 to June 2012,12 patients suffering from Galeazzi fracture-dislocations accompanied with TFCC injury were treated.In addition to open reduction and internal fixation (ORIF) of the radial shaft fracture,the DRUJ was reduced via a dorsal incision and TFCC repaired with bone anchors.Of the 12 patients 8 were male and 4 were female.Their ages ranged from 28 to 50 years,with an average of 37.5 years.All the fractures were caused by trauma and were treated within one week of injury.Results The average duration of the follow-up was 12 months (range 6 to 24 months).Of these 12 lesions,all radial shaft fractures healed.According to the evaluation of Disabilities of the Arm,Shoulder and Hand (DASH),the average score was 7.3 (range,3.3 to 12.6).Conclusion ORIF of the radial shaft fracture combined with TFCC reconstruction is an effective way for treatment of Galeazzi fracture-dislocations accompanied with TFCC
