目的探讨基层医院应用股骨近端防旋髓内钉（PFNA）治疗老年股骨粗隆间粉碎骨折的疗效。方法回顾本院2009年1月~2012年5月行在本院行 PFNA内固定的老年股骨粗隆间粉碎性骨折病例34例。按A0分型，A2型22例， A3型12例。其中合并糖尿病16例，高血压18例，冠心病4例，伤后距手术时间3～14d，平均6d。结果术后随访6~18个月，其中1例术后超过14个月未愈合行加长柄人工股骨头置换术，2例出现术后髋部疼痛，1例出现髋内翻，1例出现假体周围骨折，余均取得良好的临床疗效，无术后感染，锁钉未发现断钉现象。骨折平均愈合时间18w。结论应用股骨近端防旋髓内钉（PFNA）治疗老年股骨粗隆间粉碎骨折，符合骨折内固定要求，效果理想。
목적탐토기층의원응용고골근단방선수내정（PFNA）치료노년고골조륭간분쇄골절적료효。방법회고본원2009년1월~2012년5월행재본원행 PFNA내고정적노년고골조륭간분쇄성골절병례34례。안A0분형，A2형22례， A3형12례。기중합병당뇨병16례，고혈압18례，관심병4례，상후거수술시간3～14d，평균6d。결과술후수방6~18개월，기중1례술후초과14개월미유합행가장병인공고골두치환술，2례출현술후관부동통，1례출현관내번，1례출현가체주위골절，여균취득량호적림상료효，무술후감염，쇄정미발현단정현상。골절평균유합시간18w。결론응용고골근단방선수내정（PFNA）치료노년고골조륭간분쇄골절，부합골절내고정요구，효과이상。
Objective To explore the grass-roots hospitals application of proximal femur prevent rotation intramedul ary nail (PFNA) treatment of senile intertrochanteric femoral comminuted fracture curative ef ect. Methods Reviewed in our hospital between January 2009 and May 2012 in our hospital line PFNA internal fixation of the old age femoral intertrochanteric, 34 cases of comminuted fracture cases. According to AO classification, A2 type 22 cases, 14 cases of type A3. 18 cases among them, 16 cases with diabetes mel itus, hypertension, coronary heart disease in 4 cases, from the operation time 3 ~ 14 d after injury, an average of 6 d. Result Postoperative fol ow-up of 6 to 18 months, including 1 case of more than 14 months postoperatively GeHang not longer shank artificial femoral head replacement, 2 cases with postoperative hip pain, 1 case appeared hip varus, 1 case appeared fracture around the prosthesis, has achieved good clinical curative ef ect, no postoperative infection, no screw breakage phenomenon found locking. Average fracture healing time 18 weeks. Conclusion Application of proximal femur prevent rotation intramedul ary nail (PFNA) treatment of senile intertrochanteric femoral comminuted fracture, conform to the requirements of the internal fixation of fracture, the ef ect is ideal.
