目的探讨医院-社区-家庭一体化糖尿病管理模式干预效果。方法选取深圳市宝安区人民医院89例糖尿病患者，进行为期1.5年医院-社区-家庭一体化糖尿病综合干预，包括健康教育、行为干预、心理干预及药物干预，并进行效果评价。结果与干预前比较，干预后糖尿病危险因素知晓度、健康行为形成率、规律服药率明显提高（P＜0.01）。结论医院-社区-家庭一体化糖尿病管理模式促进了糖尿病患者相关知识、态度、行为的正向改变；患者血糖等重要指标显现初步效果。
목적탐토의원-사구-가정일체화당뇨병관리모식간예효과。방법선취심수시보안구인민의원89례당뇨병환자，진행위기1.5년의원-사구-가정일체화당뇨병종합간예，포괄건강교육、행위간예、심리간예급약물간예，병진행효과평개。결과여간예전비교，간예후당뇨병위험인소지효도、건강행위형성솔、규률복약솔명현제고（P＜0.01）。결론의원-사구-가정일체화당뇨병관리모식촉진료당뇨병환자상관지식、태도、행위적정향개변；환자혈당등중요지표현현초보효과。
Objective To investigate the hospital-Community-Family Integrated diabetes management intervention. Methods:Bao'an District, Shenzhen People's Hospital, 89 cases of patients with diabetes for a period of 1.5 years hospital - Community - Family Integration comprehensive diabetes interventions, including health education and behavioral intervention, psychological intervention and drug intervention, and for ef ect evaluation. Results: Compared with before intervention, after the intervention of risk factors for diabetes awareness degree, health behavior formation rate, regular medication was significantly increased (P<0.01). Conclusions:Hospital-Community-Family Diabetes management model to promote the integration of diabetes-related knowledge, at itudes, behaviors positive change;blood glucose and other important indicators show preliminary results.
