目的分析成纤维细胞生长因子诱导早期反应蛋白14( Fn14)在乳腺癌组织中的表达及其与临床病理因素和临床预后之间的关系。方法应用Western blot 检测20例乳腺癌及癌旁正常乳腺组织中Fn14的表达差异。应用免疫组化染色检测Fn14在113例乳腺癌中的表达模式,并通过统计学分析Fn14与临床病理因素及临床预后之间的相关性。结果在通过Western blot检测的20组配对的乳腺癌及癌旁组织中,有17组的结果显示Fn14在乳腺癌中的蛋白表达水平明显高于癌旁正常乳腺组织。在113例进行免疫组化染色的乳腺癌中,Fn14阳性患者组与Fn14阴性患者组相比,HER2过表达(++/+++)患者比例同比HER2(-/+)患者比例明显增高( P=0.010);发生淋巴结转移的患者比例同比无淋巴结转移的患者比例明显增高( P=0.015);并且TNM分期Ⅲ期的患者比例同比Ⅰ期或Ⅱ期的比例也明显增高( P=0.030)。而与患者的年龄( P=0.436)、组织学类型( P=0.421)、组织学分级( P=0.790)、肿瘤大小( P=0.403)、ER ( P=0.637)、PR(P=0.352)及Ki67指数(P=0.071)无关。通过Spearman相关性分析,我们发现Fn14与HER2的表达正相关( r =0.244, P =0.009)。另外,乳腺癌中 Fn14的表达与肿瘤的预后显著相关( Log-rank =4.603,P=0.032)。结论 Fn14的高表达可能促进乳腺癌的恶性进展。 Fn14可辅助用于乳腺癌的预后评估。

Objective To investigate the expression of fibroblast growth factor-inducible 14 ( Fn14 ) in breast carcinoma and its correlation with clinicopathological features and prognosis .Methods We examined the expression of Fn14 in 20 cases of breast carcinoma tissues and the paired normal breast tissues by Western blot analysis.The expression pattern and level of Fn14 were examined using immunohistochemistry in 113 clinicopathologically characterized breast carcinoma cases .Statistical analyses were applied to test for prognostic and diagnostic associations .Results Compared with the paired normal breast tissues , breast carcinoma tissues showed high expression of Fn14 in 17/20 cases.The result of immunohistochemical staining also showed that the average positive rate of Fn14 was increased significantly in breast cancers than those in normal breast cancer tissues . Statistical analysis of cases showed that the rate of HER 2 overexpression cases was significantly increased in Fn 14 positive group compared with Fn 14 negative group ( P=0.010 );the rate of cases with lymph node metastasis was significantly increased in Fn 14 positive group compared with Fn 14 negative group ( P=0.015 );the rate of cases in TNMⅢstage was significantly increased in Fn 14 positive group compared with Fn 14 negative group ( P=0.030 ) . However ,there was no significant difference between Fn 14 expression and the age ( P =0.436 ) , histotype ( P =0.421),grade(P=0.790),tumor size(P=0.403),ER(P=0.637),PR(P=0.352) and Ki67(P=0.071).In addition,the expression of Fn14 was positively correlated with the expression of HER 2.Conclusions Our results suggest that Fn14 might be a valuable prognostic marker for breast carcinoma and identify the patients with more aggressive feature .
