目的探讨正常高值超敏促甲状腺激素(ultra-sensitive TSH,uTSH)与血脂异常的相关性。方法纳入青岛城阳区人民医院2010年1月~2011年1月健康查体人群156例,通过电化学发光免疫分析法检测患者uTSH水平,按照uTSH界值范围分为正常低值组(0.4 mIU/L<uTSH<2.9 mIU/L,n=70)和正常高值组(3.0 mIU/L<uTSH<4.5 mIU/L,n=86)。所有研究对象检测身高、体重、腰围、臀围及血压,测定血脂、空腹血糖(FBG)、肝肾功能等指标。比较两组间上述指标的差异,并采用多元线性回归分析uTSH与血脂的关系。结果与正常低值组相比,正常高值组平均总胆固醇(TC)显著增高[(6.54±1.71)mmol/L vs.(5.76±1.54)mmol/L],差异有统计学意义(P<0.05),同时女性高TC的患病率也明显较高(40.72% vs.23.54%),差异有统计学意义(P<0.01);调整年龄、性别、体质指数(BMI)、FBG后,多元线性回归分析结果显示正常高值uTSH与TC水平具有相关性(OR=5.68,95%CI:2.69~8.13)。结论正常高值uTSH是TC水平异常的影响因素,在女性尤为显著。

목적탐토정상고치초민촉갑상선격소(ultra-sensitive TSH,uTSH)여혈지이상적상관성。방법납입청도성양구인민의원2010년1월~2011년1월건강사체인군156례,통과전화학발광면역분석법검측환자uTSH수평,안조uTSH계치범위분위정상저치조(0.4 mIU/L<uTSH<2.9 mIU/L,n=70)화정상고치조(3.0 mIU/L<uTSH<4.5 mIU/L,n=86)。소유연구대상검측신고、체중、요위、둔위급혈압,측정혈지、공복혈당(FBG)、간신공능등지표。비교량조간상술지표적차이,병채용다원선성회귀분석uTSH여혈지적관계。결과여정상저치조상비,정상고치조평균총담고순(TC)현저증고[(6.54±1.71)mmol/L vs.(5.76±1.54)mmol/L],차이유통계학의의(P<0.05),동시녀성고TC적환병솔야명현교고(40.72% vs.23.54%),차이유통계학의의(P<0.01);조정년령、성별、체질지수(BMI)、FBG후,다원선성회귀분석결과현시정상고치uTSH여TC수평구유상관성(OR=5.68,95%CI:2.69~8.13)。결론정상고치uTSH시TC수평이상적영향인소,재녀성우위현저。
Objective To discuss the correlation between high-normal range of hypersensitive thyroid stimulating hormone (TSH) and dyslipidemia. Methods The health cases (n=156) with physical examination were chosen from the People’s Hospital of Chengyang District of Qingdao City from Jan. 2010 to Jan. 2011. The level of hypersensitive TSH was detected by using electro-chemiluminescence immunoassay and all cases were divided into low-normal range group (0.4 mIU/L<uTSH<2.9 mIU/L, n=70) and high-normal range group (3.0 mIU/L<uTSH<4.5 mIU/L, n=86). The indexes were detected in all cases including height, weight, waistline, hipline, blood pressure, blood fat, fasting blood glucose (FBG) and functions of liver and kidney. The difference in these indexes was compared between two groups and the relationship between hypersensitive TSH and blood fat was analyzed by applying multivariate linear regression. Results The level of total cholesterol (TC) was significantly higher in high-normal range group than that in low-normal range group [(6.54±1.71) mmol/L vs. (5.76±1.54) mmol/L, P<0.05], and meanwhile the incidence of hypercholesteremia was significantly higher in female cases (40.72%vs. 23.54%, P<0.01). After adjusting age, sex, BMI and FBG, the results of multivariate linear regression showed that there was a correlation between high-normal hypersensitive TSH and TC level. Conclusion High-normal hypersensitive TSH is an influencing factor on TC level, which is more significant in the female.
