目的:探讨B型钠尿肽(BNP)与慢性心力衰竭患者(CHF)预后的关系。方法:收集我院128例CHF患者的临床资料,并选择同期体检的健康成人60例作为对照,分析受试者血浆BNP水平与心功能分级和左心室射血分数(LVEF)的关系,患者的住院情况和病死率。结果:随着NYHA心功能分级的增加,患者血浆BNP浓度也随之增高,二者呈正相关;患者的血浆BNP浓度与LVEF水平呈负相关;BNP诊断CHF的敏感性为90.6%,特异性为81.7%;3月内因心力衰竭再次入院患者的BNP水平显著高于未入院患者;患者的住院次数和病死率随心功能分级的增加而增加。结论:血浆BNP可以有效诊断慢性心力衰竭患者,且与患者的严重程度密切相关,可以作为判断慢性心力衰竭患者预后的指标。

목적:탐토B형납뇨태(BNP)여만성심력쇠갈환자(CHF)예후적관계。방법:수집아원128례CHF환자적림상자료,병선택동기체검적건강성인60례작위대조,분석수시자혈장BNP수평여심공능분급화좌심실사혈분수(LVEF)적관계,환자적주원정황화병사솔。결과:수착NYHA심공능분급적증가,환자혈장BNP농도야수지증고,이자정정상관;환자적혈장BNP농도여LVEF수평정부상관;BNP진단CHF적민감성위90.6%,특이성위81.7%;3월내인심력쇠갈재차입원환자적BNP수평현저고우미입원환자;환자적주원차수화병사솔수심공능분급적증가이증가。결론:혈장BNP가이유효진단만성심력쇠갈환자,차여환자적엄중정도밀절상관,가이작위판단만성심력쇠갈환자예후적지표。
Objective:To investigate the relationship of B-type natriuretic peptide (BNP) on prognosis of patients with chronic heart failure (CHF). Methods:128 cases of patients with CHF were collected and 60 cases of healthy adults as control. Analyse the relationship between plasma BNP levels of the subjects and NYHA classification and left ventricular ejection fraction (LVEF), hospitalization and mortality of the patients were compared. Results:The plasma BNP levels were increased with increasing NYHA class, and they were positively correlatedl, plasma BNP levels was negatively correlated with LVEF level. The sensitivity and specificity of BNP in CHF patients were 90.6%and 81.7%, respectively. BNP levels of readmitted patients were significantly higher than the other patients. Frequency of hospitalizations and mortality of the patients were increased with increasing NYHA class. Conclusion:Plasma BNP level is an effective indication in diagnosis of CHF, and is closely related to the severity of the patients, can be used as a indicator to determine the prognosis of patients with CHF.
