目的探讨超声造影动态观察正常实验犬前列腺射频消融灶范围及消融灶演变的可行性。方法10条雄性实验犬随机分为消融术后一周组和一个月组,每组5条。对每条犬的前列腺一侧叶行射频消融。射频术后一个月组犬,在处死前对另一侧叶行射频消融,该侧消融灶作为射频消融即刻组。定期对实验犬行经直肠常规超声、超声造影检查,同时采用超声造影测量消融灶体积。观察超声造影对前列腺消融灶的评价效果,并与最终的大体病理测量结果比较。结果射频消融后即刻、一周和一个月超声造影评价消融灶体积分别为(0.77±0.34) cm3、(1.11±0.61) cm3和(0.19±0.09) cm3。相应时期病理测量体积分别为(0.85±0.28)cm3、(0.96±0.31)cm3和(0.20±0.06)cm3。超声造影在各个时期均能准确评价消融灶的范围,其测量结果与病理结果比较无统计学差异(P>0.05),同时超声造影能准确反映消融灶及周边组织的血流分布情况。结论超声造影能反映前列腺射频消融灶的范围并能动态显示消融灶血流灌注的变化。

Objective To evaluate the lesions produced by radiofrequency ablation ( RFA) in a canine model with contrast-enhanced ultrasonography (CEUS)and investigate the feasibility to predict these lesions .Methods 10 male dogs underwent RFA of the prostate .RF was delivered under transrectal ultrasound ( TRUS ) guidance .The examinations of CEUS were performed immediately , 1 week and 1 month after RF procedure .Subsequently , all animals were sacrificed and the prostates were gotten for pathologic analysis .Results The RFA lesions volume measured by CEUS in different periods were ( 0.77 ±0.34 ) cm3 , ( 1.11 ±0.61 ) cm3 and ( 0.19 ±0.09 ) cm3 , respectively .The lesions size observed on pathological were ( 0.85 ±0.28 ) cm3 , ( 0.96 ±0.31 ) cm3 and ( 0.20 ± 0.06 ) cm3 .CEUS was able to monitor the lesions in real time .After RFA we measured the lesion′s volume with CEUS.There were no significant differences with the pathologic results .Ultrasound imaging of the ablation areas at different time had its own characteristics , which was caused by pathological transformation .Conclusion CEUS appears to be an accurate modality for assess of RFA lesions .
