目的探讨肿瘤坏死因子α基因启动子-238（TNF-α-238）基因多态性与HBV慢性感染不同临床类型的关系。方法采用聚合酶联反应-限制性片断长度多态性（ PCR-RFLP）分析法检测132例HBV慢性感染者（慢性乙型病毒性肝炎84例，肝炎肝硬化28例和慢性重型乙型肝炎20例）以及50例健康对照者的TNF-α-238基因多态性，观察其在不同临床类型慢性HBV感染者及健康对照者中的分布频率。结果慢性乙型肝炎、肝炎肝硬化、慢性重型乙型肝炎患者TNF-α-238 G/A基因型频率、等位基因频率与健康对照组比较，差异均无统计学意义（P＞0．05）。结论 HBV感染不同临床类型与TNF-α-238基因多态性无明显关系。
목적탐토종류배사인자α기인계동자-238（TNF-α-238）기인다태성여HBV만성감염불동림상류형적관계。방법채용취합매련반응-한제성편단장도다태성（ PCR-RFLP）분석법검측132례HBV만성감염자（만성을형병독성간염84례，간염간경화28례화만성중형을형간염20례）이급50례건강대조자적TNF-α-238기인다태성，관찰기재불동림상류형만성HBV감염자급건강대조자중적분포빈솔。결과만성을형간염、간염간경화、만성중형을형간염환자TNF-α-238 G/A기인형빈솔、등위기인빈솔여건강대조조비교，차이균무통계학의의（P＞0．05）。결론 HBV감염불동림상류형여TNF-α-238기인다태성무명현관계。
Objective To explore the correlation between single nucleotide polymorphism ( SNP ) of tumor necrosis factor α( TNF-α) gene promoter-238 and different clinical types of patients with chronic hepatitis B virus (HBV) infection.Methods Polymerase chain reaction-restriction fragment length polymorphism assay (PCR-RFLP) was used to detect SNP at TNF-α-238 site among 132 cases of chronic HBV infection ,including 84 cases of chronic hepatitis B (CHB),28 cases of hepatic cirrhosis (HC) and 20 cases of chronic severe hepatitis B (CSHB) ,as well as 50 healthy controls .The distribution frequencies of SNP at TNF-α-238 site were observed in patients with different clinical types of chronic HBV infection as well as in healthy controls .Results There was no significant difference in genotype frequency and allele frequency of TNF-α-238 G/A in each chronic HBV group compared with control group ( P>0 .05 ) .Conclusion Polymorphisms of TNF-α-238 might not be associated with the clinical types of patients with chronic hepatitis B .
