目的探讨咽喉真菌病的诊断和治疗。方法回顾性分析2010年3月～2012年8月我科收治的12例严重咽喉真菌病患者资料，所有患者均采用显微镜镜检及真菌培养、病理活检及内镜检查等综合方法而确诊。采用局部清洁，氟康唑静脉点滴，同时氟康唑注射液含漱；喉部真菌病除以上治疗外，加用氟康唑雾化吸入治疗。结果10例咽部严重真菌感染经积极抗真菌治疗2周后症状消失，查体见咽喉部黏膜恢复正常。1例喉真菌病支撑喉内镜下活检加病变清除，综合治疗2周明显好转出院，氟康唑50 mg口服1个月痊愈。另1例经济困难仅行综合用药治疗3周痊愈。结论真菌感染和滥用抗生素密切相关，反复多次真菌培养可减少误诊，必要时活检确诊。治疗以综合治疗为主。
목적탐토인후진균병적진단화치료。방법회고성분석2010년3월～2012년8월아과수치적12례엄중인후진균병환자자료，소유환자균채용현미경경검급진균배양、병리활검급내경검사등종합방법이학진。채용국부청길，불강서정맥점적，동시불강서주사액함수；후부진균병제이상치료외，가용불강서무화흡입치료。결과10례인부엄중진균감염경적겁항진균치료2주후증상소실，사체견인후부점막회복정상。1례후진균병지탱후내경하활검가병변청제，종합치료2주명현호전출원，불강서50 mg구복1개월전유。령1례경제곤난부행종합용약치료3주전유。결론진균감염화람용항생소밀절상관，반복다차진균배양가감소오진，필요시활검학진。치료이종합치료위주。
Objective To investigate the diagnosis and treatment of throat fungal disease. Methods The clinical data of 12 patients with severe throat fungal disease in our department from March 2010 to August 2012 were analyzed retrospectively. All patients were diagnosed definitely by the microscopic examination, fungal culture, biopsy and endoscopy and treated by local cleaning, fluconazole with intravenous and gargle. Fluconazole inhalation therapy was added in addition to the above treatment for throat fungal disease. Results The symptoms of 10 cases with serious pharyngeal fungal infection disappeared after two weeks of the active antifungal therapy and the mucous membranes of the throat got right by physical examination. The pathological lesions of 1 case with laryngeal fungal disease were cleared through laryngeal endoscopic biopsy, and the patient improved and discharged after two weeks of comprehensive treatment, and recovered by taking orally 50mg of fluconazole for 1 month. 1 case due to the economic difficulties was given comprehensive treatment only and recovered after three weeks. Conclusion Fungal infections is closely related with misuse of antibiotics. Repeated fungal culture can reduce misdiagnosis, and the biopsy should be carried out in order to further ascertain the diagnosis if necessary. The treatment methods are mainly to comprehensive treatment.
