目的:观察滑动加压万向锁定生物金属接骨板对山羊股骨愈合的影响。方法采用自身对照的方法,12只山羊的双侧股骨分别随机行滑动加压万向锁定生物金属接骨板和锁定板固定。术中保留骨折间隙2mm,术后4,8,12周分批处死动物,每批4只,其中1只截取固定骨段作组织学观察,另3只动物取出双侧股骨,行影像学观察及生物力学测试。结果①影像学观察:术后4周与8周滑动加压万向锁定生物金属接骨板组较锁定板组骨折线模糊、消失。术后12周,两种接骨板固定骨折断端均愈合良好。②力学实验:术后4周与8周滑动加压万向锁定生物金属接骨板组的抗扭力学强度均显著性大于锁定板组(配对t检验,P<0.05)。术后12周,两种接骨板的抗扭力学强度之间差异无显著性。③组织学观察:术后4周及8周时滑动加压万向锁定生物金属接骨板组骨折愈合的速度及质量均优于锁定板组,术后12周,滑动加压万向锁定生物金属接骨板组出现成熟的骨单位,锁定板组为规整的板层骨结构,哈弗氏系统排列规整,出现较多明显的骨吸收腔。结论滑动加压万向锁定生物金属接骨板对骨折断端进行持续有效加压,骨折愈合速度和质量均优于传统接骨板,具有良好的生物力学性能。

목적:관찰활동가압만향쇄정생물금속접골판대산양고골유합적영향。방법채용자신대조적방법,12지산양적쌍측고골분별수궤행활동가압만향쇄정생물금속접골판화쇄정판고정。술중보류골절간극2mm,술후4,8,12주분비처사동물,매비4지,기중1지절취고정골단작조직학관찰,령3지동물취출쌍측고골,행영상학관찰급생물역학측시。결과①영상학관찰:술후4주여8주활동가압만향쇄정생물금속접골판조교쇄정판조골절선모호、소실。술후12주,량충접골판고정골절단단균유합량호。②역학실험:술후4주여8주활동가압만향쇄정생물금속접골판조적항뉴역학강도균현저성대우쇄정판조(배대t검험,P<0.05)。술후12주,량충접골판적항뉴역학강도지간차이무현저성。③조직학관찰:술후4주급8주시활동가압만향쇄정생물금속접골판조골절유합적속도급질량균우우쇄정판조,술후12주,활동가압만향쇄정생물금속접골판조출현성숙적골단위,쇄정판조위규정적판층골결구,합불씨계통배렬규정,출현교다명현적골흡수강。결론활동가압만향쇄정생물금속접골판대골절단단진행지속유효가압,골절유합속도화질량균우우전통접골판,구유량호적생물역학성능。
Objective To observe the effect of the sliding compression universal lock biological metal plate on goat femur heal -ing.Methods Using self-control contrast method ,the bilateral femur of 12 goats were fixed respectively random by the sliding compression u-niversal lock biological metal plate and locking plate .The Intraoperative fracture gap was 2mm,the goats were partial put to death in 4,8,12 weeks,each times were 4 goats,of which only 1 were for histological observation using fixed bone segments ,the other 3 goats were for imaging observation and biomechanical test after taking bilateral femur .Results ①Imaging observation:compared with the locking plates ,the fracture line of the sliding compression universal lock biological metal plate group was fuzzy ,disappear after 4 weeks and 8 weeks,the fracture end in both kinds of bone plate was good healing after 12 weeks.②Mechanics experiment:in postoperative 4 weeks and eight weeks ,the resistance torque strength of the sliding compression universal lock biological metal plate were significantly greater than the locking plate group ( matc-hing t test,P<0.05).In postoperative 12 weeks,the resistance torque strength of two groups had no significant difference .③Histological ob-servation:in postoperative 4 weeks and 8 weeks,the fracture healing rate and the quality in the sliding compression universal lock biological metal plate group were superior to locking plate group ,in postoperative 12 weeks,the mature bone unit appeared in the sliding compression u-niversal lock biological metal plate group .The locking plate showed neat lamellar bone structure ,haversian system arrangement neat and more obvious bone resorption cavity .Conclusion For the sliding compression universal lock biological metal plate ,continuous and effective com-pression on fracture,healing,speed and quality are superior to traditional plate fracture .
