目的:探讨Ⅰ期后路病灶清除椎弓根内固定钛笼椎间植骨持续冲洗引流治疗胸椎结核的临床疗效。方法2002-01-2012-12采用Ⅰ期后路病灶清除椎弓根内固定钛笼椎间植骨持续冲洗引流治疗胸椎结核22例,其中T6-72例,T8-98例,T10-1212例。均存在脊髓神经功能损害并出现脊柱后凸畸形。结果22例患者均获随访,随访时间1~3年,所有患者术后神经症状明显改善,后凸畸形矫正,伤口均一期愈合,无慢性窦道形成,无一例内固定出现断裂,松动等现象。结论采用Ⅰ期后路病灶清除椎弓根内固定钛笼椎间植骨持续冲洗引流治疗胸椎结核,固定可靠,疗效显著。

목적:탐토Ⅰ기후로병조청제추궁근내고정태롱추간식골지속충세인류치료흉추결핵적림상료효。방법2002-01-2012-12채용Ⅰ기후로병조청제추궁근내고정태롱추간식골지속충세인류치료흉추결핵22례,기중T6-72례,T8-98례,T10-1212례。균존재척수신경공능손해병출현척주후철기형。결과22례환자균획수방,수방시간1~3년,소유환자술후신경증상명현개선,후철기형교정,상구균일기유합,무만성두도형성,무일례내고정출현단렬,송동등현상。결론채용Ⅰ기후로병조청제추궁근내고정태롱추간식골지속충세인류치료흉추결핵,고정가고,료효현저。
Objective To investigate the clinical effect of one-stage posterior debridement pedi-cle screw fixation and interbody titanium cage bone drainage in the treatment of thoracic tubercu-losis. Methods From January 2002 to December 2012,22 patients with thoracic spinal tuberculosis were treated by posterior focus debridement and pedicle screw internal fixation titanium cage bone graft drainage. 2 cases had lesion at T6-7,8 cases at T8-9 and 12 cases at T10-12. All cases were complicated with neurological deficit and kyphosis. Results All patients were followed up for 1-3 years. All cases with neurological deficit recovered to different extent, kyphosis deformity correction. All these patients had their incision healed by first intention without chronic infection or sinus for-mation. No internal fixation breakage or loosening was observed. Conclusion The one stage poste-rior focus debridement and pedicle screw internal fixation of titanium cage interbody continuous ir-rigation and drainage in the treatment of thoracic tuberculosis has reliable fixation and significant effect.
