目的：探讨中药健脾利水方联合胸腔内注入顺铂治疗恶性胸腔积液的疗效观察。方法：病理诊断的恶性胸腔积液60例，经胸腔插管引流术排尽胸液后，按随机化原则分为2组，治疗组（34例）在胸腔内注入DDP 40mg/m2，并加服中药健脾利水方加减，水煎服，每日1剂早晚分服；对照组（26例）在胸腔内注入DDP 40mg/m2。3d后重复1次。观察疗效、生活质量、生存率以及毒副反应。结果：治疗组总有效率94.1％，病变进展率2．9％，较对照组50.0％、26.5％差异有显著性（P＜0．05）。Kamofshky的评分＞70分，治疗组较对照组有显著提高（P＜0．05）。治疗组0．5、1、1.5、2年的生存率为94．12％、67．65％、55．88％、8．82％，分别高于对照组的76．92％、34．62％、19．23％、3．85％，其中0．5、1．5年的生存率差异有显著性（P＜0．05）。结论：中药健脾利水方联合胸腔内注入顺铂治疗恶性胸腔积液。安全有效，副作用低，是治疗恶性胸腔积液的理想药物。
목적：탐토중약건비이수방연합흉강내주입순박치료악성흉강적액적료효관찰。방법：병리진단적악성흉강적액60례，경흉강삽관인류술배진흉액후，안수궤화원칙분위2조，치료조（34례）재흉강내주입DDP 40mg/m2，병가복중약건비이수방가감，수전복，매일1제조만분복；대조조（26례）재흉강내주입DDP 40mg/m2。3d후중복1차。관찰료효、생활질량、생존솔이급독부반응。결과：치료조총유효솔94.1％，병변진전솔2．9％，교대조조50.0％、26.5％차이유현저성（P＜0．05）。Kamofshky적평분＞70분，치료조교대조조유현저제고（P＜0．05）。치료조0．5、1、1.5、2년적생존솔위94．12％、67．65％、55．88％、8．82％，분별고우대조조적76．92％、34．62％、19．23％、3．85％，기중0．5、1．5년적생존솔차이유현저성（P＜0．05）。결론：중약건비이수방연합흉강내주입순박치료악성흉강적액。안전유효，부작용저，시치료악성흉강적액적이상약물。
Objective :Study on jianpi Lishui recipe of traditional Chinese medicine combined with intrathoracic injection and cisplatin in the treatment of malignant hydrothorax with observation .Methods :Pathological diagnosis of malignant pleural effusion 60 cases ,the chest tube to drain pleural fluid drainage ,the principle of randomly divided into two groups ,treatment group (34 cases) in intrapleural DDP 40mg/m2 ,plus traditional Chinese medicine and health services spleen diuresis addition and subtraction ,Shuijianbi day a morning and evening hours service ;the control group (26 ca-ses) in intrapleural DDP 40mg/m2 .3 days ,repeated 1 times .Efficacy ,quality of life ,survival and toxicity .Results:Treatment group ,the total efficiency of 94.1% ,disease progress rate of 2 .9% ,compared with the control group , 50.0% ,26.5% difference was statistically significant (P<0 .05) .Kamofshky score greater than 70 points ,the treat-ment group than the control group significantly increased (P<0 .05) .0 .5 years of treatment group ,1 ,1 .5 years surviv-al rate was 94 .12% ,67 .65% ,55 .88% ,8 .82% ,respectively ,higher than 76 .92% ,34 .62% ,19 .23% ,3 .85% ,of which 0 .5 ,1 .5 years there was a significant difference in survival (P<0 .05) .Conclusion:Jianpi Lishui recipe of tradi-tional Chinese medicine combined with intrathoracic injection and cisplatin in the treatment of malignant pleural effu-sion ,safe and effective ,side effects ,is the ideal drug in treatment of malignant pleural effusion .
