目的：了解本院门诊不合理用药情况，促进临床合理用药。方法统计2011年1月至2012年12月共计10152张门诊处方，按照《处方管理办法》、《医院处方点评管理规范》、《抗菌药临床应用指导原则》、药品说明书等规定，对其中不合理用药处方进行统计分析。结果不合理用药处方532张（占5.25%），不合理情况主要有药品适应症与诊断不符、重复用药、无指征使用抗菌药物、超剂量、药物相互作用、配伍禁忌、儿童警告。结论本院门诊处方仍存在不合理用药现象，需在处方规范化管理上要加强力度，以减少不合理用药，保障患者用药安全。
목적：료해본원문진불합리용약정황，촉진림상합리용약。방법통계2011년1월지2012년12월공계10152장문진처방，안조《처방관리판법》、《의원처방점평관리규범》、《항균약림상응용지도원칙》、약품설명서등규정，대기중불합리용약처방진행통계분석。결과불합리용약처방532장（점5.25%），불합리정황주요유약품괄응증여진단불부、중복용약、무지정사용항균약물、초제량、약물상호작용、배오금기、인동경고。결론본원문진처방잉존재불합리용약현상，수재처방규범화관리상요가강력도，이감소불합리용약，보장환자용약안전。
Objective:To understand the irrational drug use in outpatient of our hospital,and promote rational drug use.Methods Statistics from January 2011 to December 2012 a total of 10,152 outpatient prescriptions, in accordance with the provisions of the "the prescription management", "hospital prescription review management practices", the clinical application of the guiding principles of antimicrobial drugs, medicines manual, including improper use of medicinesprescription for statistical analysis. Results Irrational Prescriptions 532 (5.25%), unreasonable drug indications and diagnostic inconsistent, repeated drug use, no indication for the use of antimicrobial drugs, overdose, drug interactions, incompatibility, children warning. Conclusion Hospital outpatient prescription there is stil the phenomenon of irrational drug need prescription standardized management to strengthen efforts to reduce improper use of medicines, to protect the safety of the drug in patients.
