目的：探讨改良钻孔引流术治疗慢性硬膜下血肿（ CSDH）的临床疗效。方法：患者共36例，男27例，女9例，在传统钻孔引流术的基础上扩大骨孔至2．5 cm，悬吊硬膜，引流管置于骨孔处，不缝合颞肌，利用颅内压和大气压用生理盐水将颅内积气置换出来。结果：36例中治愈29例，好转7例，总有效率100％。改良钻孔引流术治疗CSDH能有效减少血肿残留复发、颅内积气、癫痫等术后并发症。结论：改良钻孔引流术治疗CSDH具有操作简单、创伤小、引流完全等优点，可减少术后相应并发症，有利于患者预后。
목적：탐토개량찬공인류술치료만성경막하혈종（ CSDH）적림상료효。방법：환자공36례，남27례，녀9례，재전통찬공인류술적기출상확대골공지2．5 cm，현조경막，인류관치우골공처，불봉합섭기，이용로내압화대기압용생리염수장로내적기치환출래。결과：36례중치유29례，호전7례，총유효솔100％。개량찬공인류술치료CSDH능유효감소혈종잔류복발、로내적기、전간등술후병발증。결론：개량찬공인류술치료CSDH구유조작간단、창상소、인류완전등우점，가감소술후상응병발증，유리우환자예후。
Objective:To explore the clinical curative effects about improved bore-drainage treatment in chronic subdurat hematoma(CSDH).Methods:Select 36 patients with CSDH,including 27 males and 9 females, on the basis of the traditional bore-drainage treatment, make the skull hole expanded to 2.5 cm, hang dural,place the tube above the hole , not suture temporalis , replace gas from cranial cavity with 0.9%Nacl while utilize the difference between the intracranial pressure and the atmospheric pressure .Results:29 patients were cured and 7 patients were improved .Total effective rate was 100%.Improved bore-drainage treatment can effectively reduce the hematoma's residual and recurrence , epilepsy, gas, etc.Conclusion: Improved bore-drainage treatment is simply, diminutive trauma,drainage fully, etc, can obviously improve prognosis .
