目的探讨超声弹性成像技术中弹性应变率比值对判断乳腺良恶性疾病的应用价值。方法超声弹性成像检查54个乳腺病灶,与术后病理结果对照,分析乳腺良恶性病变弹性参数的差异,从而确定良恶性病灶的弹性应变率比值的临界值。结果54个乳腺病灶中,良性病灶44个,恶性病灶10个。弹性应变率比值良性病灶为1.03±0.46,恶性病灶为1.65±1.28,后者大于前者,差异有统计学意义(P<0.05)。以乳腺病灶弹性应变率比值≥1.35作为恶性病灶的诊断临界值,敏感性为88.89%,特异性为95.56%,准确性为94.44%,阳性预测值80.01%。结论弹性应变率比值对乳腺肿块的良恶性鉴别具有较高的应用价值,为超声检查提供客观的诊断依据。

Objective To investigate the application value of ultrasonic elastography strain ratio in breast benign and malignant lesions. Methods Elasticity imaging was used to examine 54 breast lesions,the parameters on elasticity imaging were retrospectively reviewed and compared with pathology,and the cut-off point of strain ratio between benign and malignant lesions was determined. Results There were 44 benign and 10 malignant lesions. The strain ratio lesions was 1.03 ±0.46 for benign lesions, and 1.65±1.28 for malignant lesions, there was significant difference between the strain ratio of benign and malignant lesions (P<0.05).The cut-off point of strain ratio was determined as 1.35 for malignant lesions, the sensitivity was 88.89%, specificity was 95.56%,accuracy was 94.44%, and positive predictive value was 80.01%.Conclusion Strain ratio has higher diagnosis value for differentiating breast benign and malignant lesions, and provides objective diagnosis basis for ultrasound.
