目的探讨彩色多普勒超声对肝豆状核变性(HLD)患者肾脏血流动力学改变的诊断价值。方法30例体检健康者为对照组,61例HLD患者分为肾脏回声改变组(A组)和肾脏回声无改变组(B组),彩色多普勒超声检测其三级肾动脉(肾主动脉、段间动脉、叶间动脉),计算三级动脉的显示率,测量收缩期最大血流速度和阻力指数。结果对照组和B组肾主动脉、段间动脉及叶间动脉显示率均为100%。A组肾主动脉显示率为100%,段间动脉显示率为84.3%,叶间动脉显示率为71.8%。与对照组比较,A组和B组的肾主动脉、肾段间动脉及叶间动脉血流速度减慢,阻力指数增高,差异均有统计学意义(P<0.05)。结论 HLD患者肾脏血管血流动力学状态可发生明显改变。

목적탐토채색다보륵초성대간두상핵변성(HLD)환자신장혈류동역학개변적진단개치。방법30례체검건강자위대조조,61례HLD환자분위신장회성개변조(A조)화신장회성무개변조(B조),채색다보륵초성검측기삼급신동맥(신주동맥、단간동맥、협간동맥),계산삼급동맥적현시솔,측량수축기최대혈류속도화조력지수。결과대조조화B조신주동맥、단간동맥급협간동맥현시솔균위100%。A조신주동맥현시솔위100%,단간동맥현시솔위84.3%,협간동맥현시솔위71.8%。여대조조비교,A조화B조적신주동맥、신단간동맥급협간동맥혈류속도감만,조력지수증고,차이균유통계학의의(P<0.05)。결론 HLD환자신장혈관혈류동역학상태가발생명현개변。
Objective To investigate the diagnostic value of color Doppler ultrasound for renal hemodynamic changes in hepatolenticular degeneration(HLD) patients. Methods Thirty healthy volunteers were served as control group, 61 patients with HLD were divided into HLD nephropathy group (group A) and HLD group with no changes in renal echo (group B) . Their master renal arteries (MRA),segmental renal arteries(SRA)and interlobular renal arteries(IRA)were detected by color Doppler ultrasound. The display rate of the arteries were calculated, the peak systolic velocity (PSV) and resistant index (RI)were measured. Results The display rate of MRA, SRA and IRA in control group and group B was 100%. The display rate of MRA, SRA and IRA in group A was 100%, 84.3% and 71.8%, respectively. Compared with control group, the PSV of MRA, SRA and IRA was decreased in group A and B, RI was increased. These differences had statistical significance (P < 0.05). Conclusion The renal hemodynamics has significant changes in HLD patients.
