目的:探讨QT离散度在预测急性心肌梗死患者室性心律失常的临床应用价值。方法:回顾性分析47例急性心肌梗死未合并室性心律患者及42例急性心肌梗死合并室性心律患者心电图检查资料,比较两组患者QT离散度、校正QT离散度指标,分析校正QT离散度水平与室性心律失常发生率之间的关系。结果:观察组患者QT离散度(71.4±13.5)及校正QT离散度(80.9±18.0)均明显高于对照组(P<0.05);校正QT离散度水平大于等于60ms急性心肌梗死患者室性心律失常发生率(97.4%)明显高于校正QT离散度水平小于60ms急性心肌梗死患者(P<0.05)。结论:急性心肌梗死患者QT离散度、校正QT离散度越高室性心律失常发生风险越高;校正QT离散度可作为急性心肌梗死患者早期室性心律失常预测的敏感指标。

목적:탐토QT리산도재예측급성심기경사환자실성심률실상적림상응용개치。방법:회고성분석47례급성심기경사미합병실성심률환자급42례급성심기경사합병실성심률환자심전도검사자료,비교량조환자QT리산도、교정QT리산도지표,분석교정QT리산도수평여실성심률실상발생솔지간적관계。결과:관찰조환자QT리산도(71.4±13.5)급교정QT리산도(80.9±18.0)균명현고우대조조(P<0.05);교정QT리산도수평대우등우60ms급성심기경사환자실성심률실상발생솔(97.4%)명현고우교정QT리산도수평소우60ms급성심기경사환자(P<0.05)。결론:급성심기경사환자QT리산도、교정QT리산도월고실성심률실상발생풍험월고;교정QT리산도가작위급성심기경사환자조기실성심률실상예측적민감지표。
Objective:To investigate the clinical application of QTd dispersion in predicting acute myocardial infarction in patients with ventricular arrhythmias.Methods:The ECG data of 47 acute myocardial infarction patients without ventricular arrhythmias and 42 acute myocardial infarction patients with ventricular arrhythmias were analyzed;the QTd and QTcd level of two groups were compared,the relationship between QTcd level and ventricular arrhythmias incidence were analyzed.Results:The QTd(71.4±13.5) and QTcd(80.9±18.0) of observation group were significantly higher than control group(P<0.05). The ventricular arrhythmias incidence of acute myocardial infarction patients with QTcd level greater than and equal to 60mm (97.4%)were significantly higher than acute myocardial infarction patients with QTcd level shorter than 60mm(P<0.05).Conclusion:Acute myocardial infarction patients with higher QTd and QTcd level have high risk of ventricular arrhythmias. QTcd can be defined as the sensitive indicator of predicting acute myocardial infarction in patients with ventricular arrhythmias.
