目的进一步探讨糖尿病(DM)患者糖化血红蛋白(GHb)检测的临床价值。方法分别采用免疫透射比浊法和葡萄糖氧化酶法测定糖化血红蛋白A1C(HbA1C)与空腹血糖(FBG)。结果DM患者组HbA1C、FBG结果与健康对照组差异有统计学意义(P<0.001);DM患者组治疗前FBG结果与治疗后差异有统计学意义(P<0.001);DM患者组治疗前HbA1C结果与治疗后比较,HbAlC结果4周内下降缓慢(P>0.05),其下降幅度明显低于FBG,治疗5周后HbAlC才明显降低(P<0.001),至10周后HbAlC接近正常水平。结论HbAlC测定可反映大约前1~2个月的血糖平均水平,可作为DM治疗疗效的监测指标;与FBG联合检测,对DM的筛查、诊断和鉴别诊断、控制情况和并发症的预防具有非常重要的意义。

목적진일보탐토당뇨병(DM)환자당화혈홍단백(GHb)검측적림상개치。방법분별채용면역투사비탁법화포도당양화매법측정당화혈홍단백A1C(HbA1C)여공복혈당(FBG)。결과DM환자조HbA1C、FBG결과여건강대조조차이유통계학의의(P<0.001);DM환자조치료전FBG결과여치료후차이유통계학의의(P<0.001);DM환자조치료전HbA1C결과여치료후비교,HbAlC결과4주내하강완만(P>0.05),기하강폭도명현저우FBG,치료5주후HbAlC재명현강저(P<0.001),지10주후HbAlC접근정상수평。결론HbAlC측정가반영대약전1~2개월적혈당평균수평,가작위DM치료료효적감측지표;여FBG연합검측,대DM적사사、진단화감별진단、공제정황화병발증적예방구유비상중요적의의。
Objective To further explore the clinical value of detecting HbA1C of diabetes mellitus (DM) patients. Methods Glycated hemoglobin A1C (HbA1C) and fasting blood glucose (FBG) were detected respectively by using immunity transmission turbidity and glucose oxidase method. Results HbA1C, FBG in DM group had more significant difference than those in control group (P<0.001). There were significant differences of FBG in DM group before and after treatment (P<0.001); HbA1C in DM group after treatment fell more slowly than that before treatment in 4 weeks (P>0.05), its decline was significantly lower than FBG. HbAlC only decreased significantly five weeks after treatment (P<0.001), and the levels of HbAlC returned to normal after 10 weeks. Conclusion HbAlC determination can reflect the average level of blood sugar at the first 1~ 2 months, and it can be used as an indicator for monitoring DM therapy. It is very important significance for DM screening, diagnosis and differential diagnosis, control situation and prevention complications to joint detection with FBG.
