目的探讨腹腔镜胃旁路手术治疗2型糖尿病的临床效果及治疗体会。方法分析我院2010年8月-2012年2月开展腹腔镜胃旁路手术治疗糖尿病的20例病例资料,观察患者手术前后空腹血糖、体重指数、胰岛素抵抗(HOMA-IR)、糖化血红蛋白(HbAlc)、c肽等指标的改变。结果20例患者术后1个月血糖均有明显下降,葡萄糖耐量明显增强(P<0.05),12例患者术后1个月体重指数变化明显,15例术后随访3-12月,血糖在正常范围,不需胰岛素或药物控制;4患者例术后6个月, HOMA-IR、HbAlc与手术前相比明显降低(P<0.05);糖尿病相关并发症均有不同程度缓解。结论胃旁路术能有效治疗2型糖尿病,手术安全可行,近期效果明显,长期效果仍待观察。

목적탐토복강경위방로수술치료2형당뇨병적림상효과급치료체회。방법분석아원2010년8월-2012년2월개전복강경위방로수술치료당뇨병적20례병례자료,관찰환자수술전후공복혈당、체중지수、이도소저항(HOMA-IR)、당화혈홍단백(HbAlc)、c태등지표적개변。결과20례환자술후1개월혈당균유명현하강,포도당내량명현증강(P<0.05),12례환자술후1개월체중지수변화명현,15례술후수방3-12월,혈당재정상범위,불수이도소혹약물공제;4환자례술후6개월, HOMA-IR、HbAlc여수술전상비명현강저(P<0.05);당뇨병상관병발증균유불동정도완해。결론위방로술능유효치료2형당뇨병,수술안전가행,근기효과명현,장기효과잉대관찰。
Objective To evaluate the surgical approach and effectiveness of laparoscopic Roux-en-Y gastric bypass(LRYGB) surgery for treating type 2 diabetes mellitus. Method To analyze 20 patients' data with laparoscopic Roux-en-Y gastric bypass surgery for treatment of type 2 diabetes mellitus from August 2010 to February 2012. Result The blood glucose of 20 patients were reduced significantly.Glucose tolerance enhanced(P<0.05). The body mass of 12 patients were changed significantly. 15 patients were visited from 3 month to 12 month, the blood glucose in normal range without taking up insulin or medications. The cases of 4 patients were visited for 6 months, HOMA-IR, HbAlc compared with preoperative significantly reduced(P<0.05); Diabetes-related complications have varying degrees of relief. Conclusion LRYGB surgery is feasible for treatment of type 2 diabetes with good short-term result, but the long-term outcome awaits further observation.
