目的探讨胰腺实性假乳头状瘤的CT特征性表现.方法回顾性分析经病理证实的胰腺实性假乳头状瘤患者49例病例资料,分析其CT征象,并与临床病理进行对照.结果所有病例均为单发病灶,病变位于胰头7例,胰体23例,胰体尾15例,胰尾4例.42例患者为女性.瘤细胞围绕纤维血管轴心排列成乳头状是其病理的特征性表现.CT显示病灶囊实相间,增强后实性部分动脉期不同程度强化,强化呈渐进性,可见包膜强化.囊性部分不强化.病变边界清楚42例.病灶内可见钙化灶11例.结论胰腺实性假乳头状瘤CT表现有其特征性,临床上需加以注意以提高术前正确诊断率.

Objective To analyze the characteristic CT signs in solid pseudopapillary tumor of the pancreas(SPTP).Methods Clinical data of 49 SPTP patients were retrospectively analyzed.CT signs were compared with pathological characteristics.Results All cases were single lesions including 7 cases in pancreatic head,23 cases in pancreatic body,15 cases in pancreatic body-tail,and 4 cases in pancreatic tail.42 cases were female.Histological features were psudopapillary structure with a fibrovascular core.CT showed that the tumors were composed of solid and polycystic parts and varying degrees of contrast enhancement of the solid components in the arterial phase.The contrast enhancement gradually in the venous and delayed phase.The boundaries of the lesions were clear in 42 cases.Calcifications were found in 11 cases.Conclusions CT manifestations of solid pseudopapillary tumor of the pancreas are specific,which are helpful in the establishment of the diagnosis of this disease.
