目的:观察鼻塞式持续正压通气(BNCPAP)治疗新生儿呼吸困难的效果。方法选取2011年10月至2013年10月入院至东华医院新生儿科,不同病因合并有呼吸困难的新生儿、早产儿患儿62例,使用鼻塞式持续正压通气进行治疗,对治疗1h和24h后血气分析结果与治疗前比较,进行统计学分析。结果62例患儿中24小时后有效55例,无效7例,有效率为88.7%。有效55例中,治疗24h后临床症状如气促、呼吸困难、紫绀等均有不同程度改善,与治疗前比较,差异有统计学意义(P<0.05)。结论鼻塞式持续正压通气使用方便、无须插管、副作用小,可对新生儿及早产儿各种原因导致的呼吸困难提供有效的呼吸支持,可减少或避免气管插管机械通气带来的风险和开支。

목적:관찰비새식지속정압통기(BNCPAP)치료신생인호흡곤난적효과。방법선취2011년10월지2013년10월입원지동화의원신생인과,불동병인합병유호흡곤난적신생인、조산인환인62례,사용비새식지속정압통기진행치료,대치료1h화24h후혈기분석결과여치료전비교,진행통계학분석。결과62례환인중24소시후유효55례,무효7례,유효솔위88.7%。유효55례중,치료24h후림상증상여기촉、호흡곤난、자감등균유불동정도개선,여치료전비교,차이유통계학의의(P<0.05)。결론비새식지속정압통기사용방편、무수삽관、부작용소,가대신생인급조산인각충원인도치적호흡곤난제공유효적호흡지지,가감소혹피면기관삽관궤계통기대래적풍험화개지。
Objective: To observe the bubble nasal continuous positive airway pressure (BNCPAP) the effect of the treatment of neonatal respiratory difficulties. Methods: October 2011 to October 2013 admitted to Tung Wah Hospital neonatology, 62 cases of newborns associated with neonatal respiratory difficulties were treated by bubble nasal continuous positive airway pressure, blood gas analysis after 1 hour and 24 hours treat compared with before treatment. Results: after and 24 hours treatment, the 62 cases of newborns, the effective cases were 55,invalid cases were 7, the effective rate was 88.7%. Among effective 55 cases, the clinical symptoms such as shortness of breath, difficulty breathing, cyanosis varying degrees, after 24h treatment,were improved compared with before treatment, the difference was statistical y significant (P <0.05). Conclusions: The bubble nasal continuous positive airway pressure is easy to use, does not require endotracheal intubation, fewer complications, neonates and premature infants can be provided effective respiratory support for al causes breathing difficulties, can be reduced or avoided the risk of endotracheal intubation, and reduced the expenses.
