目的:应用超声检测评价成人睡眠呼吸暂停综合征(SAS)与颈动脉病变的关系。方法彩色多普勒超声检查176例成人SAS患者与40名健康对照组的颈总动脉内中膜厚度(IMT)、颈动脉粥样硬化斑块、管腔狭窄等指标。结果①SAS组患者IMT较对照组显著增厚[(1.30±0.27)mm>(1.05±0.21)mm,P<0.05];②SAS组检测出粥样硬化斑块形成62例(35.2%)、管腔狭窄47例(26.7%);对照组分别为5例及4例(12.5%、10.0%),2组比较差异有统计学意义(P<0.01);③IMT与夜间最低血氧(LSaO2)呈正相关(r=0.46,P<0.05),与AHI无明显相关。结论颈动脉超声能较好评价SAS血管病变。夜间缺氧会增加SAS患者颈动脉病变发生率及程度。

목적:응용초성검측평개성인수면호흡잠정종합정(SAS)여경동맥병변적관계。방법채색다보륵초성검사176례성인SAS환자여40명건강대조조적경총동맥내중막후도(IMT)、경동맥죽양경화반괴、관강협착등지표。결과①SAS조환자IMT교대조조현저증후[(1.30±0.27)mm>(1.05±0.21)mm,P<0.05];②SAS조검측출죽양경화반괴형성62례(35.2%)、관강협착47례(26.7%);대조조분별위5례급4례(12.5%、10.0%),2조비교차이유통계학의의(P<0.01);③IMT여야간최저혈양(LSaO2)정정상관(r=0.46,P<0.05),여AHI무명현상관。결론경동맥초성능교호평개SAS혈관병변。야간결양회증가SAS환자경동맥병변발생솔급정도。
Objective To discuss the association between the carotid artery lesions and adult sleep apnea syn-drome (SAS) by ultrasound. Methods Compared with 40 healthy adult controls, 176 adult SAS patients were exam-ined with color Doppler ultrasound and carotid artery intima-media thickness (IMT), carotid artery plaque and vascular constriction were measured. Results ① The carotid IMTs were statistically significant increased in the SAS patients [(1.30 ±0.27)mm] compared with the control group [(1.05 ±0.21)mm] (P<0.05). ② The SAS patients had higher preva-lence of carotid artery plaque and vascular constriction than the control group. ③The lowest arterial oxygen saturation (LSaO2) was significantly associated with IMT (r=0.46,P<0.05), but no significant correlation was found between apnea hypopnea index(AHI) and IMT. Conclusion The color Doppler ultrasound of carotid is one of the important checks to evaluate the artery lesions in SAS patients, and LSaO2 is contributing to the artery lesions.
