目的探讨新生儿缺氧缺血性脑病(HIE)的CT表现。方法对326例临床诊断为HIE及颅内出血的新生儿进行头颅CT扫描,主要采用5 mm×5 mm层厚、间距法;部分病例选择第2~8层采用5 mm×5 mm层厚、间距,其他层面采用10 mm×10 mm层厚、间距的方法进行扫描。并对CT表现进行分析。结果本组241例表现出不同程度的HIE,累及额叶最为多见,共225例,其中单纯累及额叶45例,累及2个以上脑叶180例;多脑叶弥散性低密度影16例;合并脑实质及蛛网膜下腔小量出血86例(大面积出血病例被除外)。分度为Ⅰ级35例,Ⅱ级157例,Ⅲ级36例,Ⅳ级13例。结论 CT值下降是诊断HIE的重要依据之一。
目的探討新生兒缺氧缺血性腦病(HIE)的CT錶現。方法對326例臨床診斷為HIE及顱內齣血的新生兒進行頭顱CT掃描,主要採用5 mm×5 mm層厚、間距法;部分病例選擇第2~8層採用5 mm×5 mm層厚、間距,其他層麵採用10 mm×10 mm層厚、間距的方法進行掃描。併對CT錶現進行分析。結果本組241例錶現齣不同程度的HIE,纍及額葉最為多見,共225例,其中單純纍及額葉45例,纍及2箇以上腦葉180例;多腦葉瀰散性低密度影16例;閤併腦實質及蛛網膜下腔小量齣血86例(大麵積齣血病例被除外)。分度為Ⅰ級35例,Ⅱ級157例,Ⅲ級36例,Ⅳ級13例。結論 CT值下降是診斷HIE的重要依據之一。
목적탐토신생인결양결혈성뇌병(HIE)적CT표현。방법대326례림상진단위HIE급로내출혈적신생인진행두로CT소묘,주요채용5 mm×5 mm층후、간거법;부분병례선택제2~8층채용5 mm×5 mm층후、간거,기타층면채용10 mm×10 mm층후、간거적방법진행소묘。병대CT표현진행분석。결과본조241례표현출불동정도적HIE,루급액협최위다견,공225례,기중단순루급액협45례,루급2개이상뇌협180례;다뇌협미산성저밀도영16례;합병뇌실질급주망막하강소량출혈86례(대면적출혈병례피제외)。분도위Ⅰ급35례,Ⅱ급157례,Ⅲ급36례,Ⅳ급13례。결론 CT치하강시진단HIE적중요의거지일。
Objective In neonatal hypoxic ischemic encephalopathy (HIE)CT. Methods The clinical diagnosis of 326 cases were examined by CT scan and HIE and neonatal intracranial hemorrhage in newborn,mainly uses the 5X5mm layer thickness,spacing method,in some cases second~8 layer using 5 mm×5 mm layer thickness,spacing, and other aspects of the 10 mm ×10 mm layer thickness,spacing method scan. And the CT features were analysed, finally 241 cases showed different degrees of HIE. Results This group of 241 cases involving the frontal lobe is the most common, a total of 225 cases,of which only involving the frontal lobe in 45 cases, 180 cases involving more than two lobes of the brain;16 cases of diffuse low density multi-lobes of the brain; the combined brain parenchyma and cobwebsmall subarachnoid hemorrhage 86 cases (cases of major hemorrhage were excluded). Indexing grade Ⅰ 35 cases;157 cases of grade II;36 cases of gradeⅢ;13 cases of gradeⅣ.Conclusion The decrease of CT is one of the important basis for the diagnosis of HIE.
