目的评价帕瑞昔布钠复合芬太尼在骨折术后静脉镇痛中的应用效果及安全性。方法60例择期行骨科切开复位内固定手术患者按骨折类型均分为P、C 2组,P组分别在术前15 min、术后2 h静注帕瑞昔布钠40 mg,术毕2组均采用芬太尼静脉自控镇痛泵进行术后镇痛。记录2组术后5,7,12,24 h视觉模拟评分(VAS),镇痛泵按压次数,术后5,7,12,24 h芬太尼用量和不良反应。结果 P组术后5,7,12,24 h VAS评分、镇痛泵按压次数、术后5,7,12,24 h芬太尼用量均少于C组(P<0.05)。2组不良反应无显著差异。结论帕瑞昔布钠复合芬太尼用于骨科骨折手术术后静脉自控镇痛泵(PCA)效果肯定,减少了芬太尼的用量,不良反应少,适合临床推广。

목적평개파서석포납복합분태니재골절술후정맥진통중적응용효과급안전성。방법60례택기행골과절개복위내고정수술환자안골절류형균분위P、C 2조,P조분별재술전15 min、술후2 h정주파서석포납40 mg,술필2조균채용분태니정맥자공진통빙진행술후진통。기록2조술후5,7,12,24 h시각모의평분(VAS),진통빙안압차수,술후5,7,12,24 h분태니용량화불량반응。결과 P조술후5,7,12,24 h VAS평분、진통빙안압차수、술후5,7,12,24 h분태니용량균소우C조(P<0.05)。2조불량반응무현저차이。결론파서석포납복합분태니용우골과골절수술술후정맥자공진통빙(PCA)효과긍정,감소료분태니적용량,불량반응소,괄합림상추엄。
Objective To investigate and evaluate the efficacy of parecoxib sodium combined with fentanyl in postoperative department of orthopedics operation effects and safety. Methods 60 patients undergoing open reduction and internal fixation operation in Department of orthopedics patients, 60 patients according to the fracture types were P, C two group, P group respectively in the preoperative 15min 2h after intravenous injection of parecoxib sodium 40 mg, postoperative patient-controlled intravenous analgesia with fentanyl for postoperative analgesia pump. Recorded after 5 , 7, 12, 24hVAS score, analgesic pump pressing times, after 5, 7, 12, 24h fentanyl dosage and adverse reactions. Results In P group, after 5, 7, 12, 24hVAS score, analgesic pump pressing times, after 5, 7, 12, 24h fentanyl dosage was less than C group (P<0.05). No significant differences in adverse reactions of the two groups. Conclusion Parry Celebrex sodium combined with fentanyl for patient-controlled intravenous analgesia pump Department of orthopedics fracture operation after operation (PCA) effect, reduces the dosage of fentanyl, less adverse reaction, is suitable for the application of fracture operation in Department of orthopedics.
