目的对比研究应用氯沙坦和一般治疗对肺源性心脏病患者右心功能的影响。方法将78例肺心病患者随机分为两组。对照组（n=39）给予肺心病常规治疗，氯沙坦组（n=39）给予常规治疗基础上加用氯沙坦治疗。于治疗前及治疗后6周分别测定肺通气功能、用力肺活量（FVC）、ls用力呼气容积（FEV1）和最大呼气流量（PEF）及超声测定右室流出道内径、右心室内径、右心肺动脉内径。结果氯沙坦治疗后肺功能明显改善，右心内径及肺动脉内径，右室流出道内径小于治疗前，亦明显小于一般治疗组治疗后。结论氯沙坦不仅能改善肺通气功能，亦能明显改善肺血管及右心室重构，减轻肺血管阻力。
목적대비연구응용록사탄화일반치료대폐원성심장병환자우심공능적영향。방법장78례폐심병환자수궤분위량조。대조조（n=39）급여폐심병상규치료，록사탄조（n=39）급여상규치료기출상가용록사탄치료。우치료전급치료후6주분별측정폐통기공능、용력폐활량（FVC）、ls용력호기용적（FEV1）화최대호기류량（PEF）급초성측정우실류출도내경、우심실내경、우심폐동맥내경。결과록사탄치료후폐공능명현개선，우심내경급폐동맥내경，우실류출도내경소우치료전，역명현소우일반치료조치료후。결론록사탄불부능개선폐통기공능，역능명현개선폐혈관급우심실중구，감경폐혈관조력。

Results Losartan significantly improved lung function, right ventricular diameter and pulmonary artery, right ventricular outflow tract diameter is less than before treatment, but also significantly less than the general group after treatment. Conclusion Losartan not only improve pulmonary function also improved pulmonary vascular and right ventricular remodeling and reduce pulmonary vascular resistance.
