目的探讨糖尿病患者血清 Ghrelin水平与肥胖、胰岛素抵抗之间的关系。方法将300例2型糖尿病患者根据BMI分为体重过低、体重正常、超重和肥胖4组;采用ELISA法测定血清中 Ghrelin水平变化;用直线相关及多元逐步回归分析血清Ghrelin与上述指标的相关性。结果糖尿病肥胖组的Ghrelin水平显著低于糖尿病体重过低组、体重正常组和超重组(p<0.05),而且随着BMI增加而减少;糖尿病患者血清Ghrelin与BMI、HOMA-IR、FINS 和WHR呈显著负相关(r=-0.728, p<0.01;r=-0.562, p<0.01;r=-0.490, p<0.01;r=-0.389, p<0.01);BMI是影响血清Ghrelin水平的最重要变量。结论 BMI是糖尿病患者血清Ghrelin水平的主要相关因素;Ghrelin参与了体脂形成和胰岛素抵抗的变化,在糖尿病的发病过程中起着重要作用。

Objective To investigate the relationships of the serum Ghrelin levels and obesity,insulin resistance. Methods 300 type 2 diabetic mel itus cases were divided into 4 groups according to BMI,which were named by underweight group,normal weight group, overweight group and obese group,respectively.The 1evels of Ghrelin was measured by ELISA.The relationships among serum Ghrelin concentrations with al the indexes were explored by linear correlation and multiple stepwise regression correlation analysis.Results In the obese diabetes, the serum ghrelin was significantly lower than that of the other three groupsdiabetes (p<0.05) and was decreased with the increment of BMI.The serum ghrelin of the diabetes was negatively correlated with BMI,HOMA-IR,FINS and WHR(r=-0.728, p<0.01;r=-0.562, p<0.01;r=-0.490, p<0.01;r=-0.389, p<0.01). The BMI seemed to be the best variable in predicting serum ghrelin levels.Conclusion The BMI is the determinative factor in serum ghrelin level ,and Ghrelin takes part in body fat formation and changes of insulin resistance which may indicate that Ghrelin plays important role in the pathogenesis of diabetes mel itus.
