目的探讨CT引导下125 I放射性粒子植入术治疗腹膜后淋巴结转移的临床应用及效果。方法对25例腹膜后淋巴结转移患者行CT引导下125 I放射性粒子植入术,先利用TPS系统制订治疗计划,然后在CT引导下行125 I放射性粒子植入,植入后CT扫描观察粒子分布情况,之后每月CT复查,观察病灶大小变化,并记录患者疼痛情况变化,分析治疗效果。结果25例中5例肿瘤完全缓解,11例肿瘤局部部分缓解,5例病灶无变化,4例因局部进展并其他部位转移死亡,局部控制率为64%。疼痛缓解明显。结论 CT引导下125 I放射性粒子植入术治疗腹膜后淋巴结转移安全、有效,可以有效控制肿瘤生长,减轻腹部疼痛,改善患者生活质量,值得临床推广应用。

목적탐토CT인도하125 I방사성입자식입술치료복막후림파결전이적림상응용급효과。방법대25례복막후림파결전이환자행CT인도하125 I방사성입자식입술,선이용TPS계통제정치료계화,연후재CT인도하행125 I방사성입자식입,식입후CT소묘관찰입자분포정황,지후매월CT복사,관찰병조대소변화,병기록환자동통정황변화,분석치료효과。결과25례중5례종류완전완해,11례종류국부부분완해,5례병조무변화,4례인국부진전병기타부위전이사망,국부공제솔위64%。동통완해명현。결론 CT인도하125 I방사성입자식입술치료복막후림파결전이안전、유효,가이유효공제종류생장,감경복부동통,개선환자생활질량,치득림상추엄응용。
Objective To study the clinical value and efifcacy of CT guided interstitial implantation of radioactive 125 I seeds for the therapy of retroperitoneal lymph node metastases. Methods 25 cases with retroperitoneal lymph node metastases were treated with CT guided interstitial implantation of radioactive 125 I seeds by TPS. Post operative CT review scans were performed immediately after implantation. The effect was followed-up by CT scans repeated every month post operation. The change of pain was recorded. Results The seeds of 25 cases were distributed successfully in the retroperitoneal lymph node metastases. The total effect rates in short period were 64%(16/25), CR 20%(5/25), PR 44%(11/25), NC 20%(5/25), PD 16%(4/25). The easement of pain was obviously. Conclusion It is safe and effective that CT guided interstitial implantation of radioactive 125 I seeds for the therapy of retroperitoneal lymph node metastases . This method can effectively control the growth of tumor, reduce abdominal pain, improve the quality of life of patients, is worthy of clinical application.
