滑车上神经为三叉神经额神经的分支，经滑车上方从内眶出孔，在皱眉肌外侧和穿经皱眉肌后穿过额肌到达头皮，分布于额部皮肤，和眶上神经一起建立额部皮肤的浅表感觉。其支配额部皮肤范围较广，且有恒定的滑车上动脉穿支营养，对额部皮肤的血供和感觉维持具有重要意义。以其为蒂的额旁正中滑车上神经营养血管皮瓣具有以下优点：（1）手术在同一部位进行，一次完成，操作简单快捷；（2）不损伤主干血管，对供区血供破坏少；（3）皮瓣带有皮神经，在受区进行吻合，可恢复感觉功能；（4）设计灵活，可沿皮神经任意切取皮瓣，能满足大部鼻缺损的需要。在为接受鼻缺损修复术的患者重塑鼻部外形的同时，消除了传统皮瓣所引起的皮肤感觉缺失，皮瓣发红等诸多不良反应，是鼻缺损修复术中的一大亮点，值得引起相关学者的关注。
