目的:探讨B型钠尿肽(B-type natriuretic peptide,BNP)和血β2微球蛋白与慢性心肾综合征的相关性,为临床诊断提供理论依据。方法:选择132例慢性心力衰竭患者为研究对象,根据有无肾功能损害将其分为单纯心衰组(73例)和心肾综合征组(59例)。测量两组患者血液中BNP及β2微球蛋白的浓度。结果:同单纯心衰组相比,心肾综合征组患者BNP与血β2微球蛋白的浓度均明显升高,分别为(438±262)pg/ml vs(584±219)pg/ml(P<0.05),(1.81±0.57)mg/L VS(3.78±0.83)mg/L(P<0.01)。结论:联合检测BNP和血β2微球蛋白浓度升高有利于慢性心功能不全早期发现肾功能损害,指导临床治疗。

목적:탐토B형납뇨태(B-type natriuretic peptide,BNP)화혈β2미구단백여만성심신종합정적상관성,위림상진단제공이론의거。방법:선택132례만성심력쇠갈환자위연구대상,근거유무신공능손해장기분위단순심쇠조(73례)화심신종합정조(59례)。측량량조환자혈액중BNP급β2미구단백적농도。결과:동단순심쇠조상비,심신종합정조환자BNP여혈β2미구단백적농도균명현승고,분별위(438±262)pg/ml vs(584±219)pg/ml(P<0.05),(1.81±0.57)mg/L VS(3.78±0.83)mg/L(P<0.01)。결론:연합검측BNP화혈β2미구단백농도승고유리우만성심공능불전조기발현신공능손해,지도림상치료。
Objective:To explore the diagnosis value of B-type natriuretic peptide(BNP)and serumβ2 microglobulin in chronic cardiorenal syndrome.Method:Totally 132 patients with chronic heart failure were enrolled.Those without kidney dysfunction were enrolled into a group named simple heart failure group(73 cases)and those with kidney dysfunction were assigned into the group named cardiorenal syndrome group(59 cases).The blood concentration of BNP and serumβ2 microglobulin were measured.Result:Compared with patients of simple heart failure group,those with cardiorenal syndrome had a higher level of BNP and serumβ2 microglobulin,(438±262)pg/mL vs(584±219)pg/mL(P<0.05),(1.81±0.57) mg/L VS(3.78±0.83)mg/L(P<0.01)separately.Conclusion:The level of BNP and serumβ2 microglobulin are closely related with chronic cardiorenal syndrome,The combined detection of BNP and serum β2 microglobulin concentrations could be helpful to detect chronic heart failure with kidney dysfunction in the early stage.
